Indeterminate statement syndrome is a group of indeterminate physical symptoms such as headache, palpitation, insomnia, precordial pain, dyspnea, gastrointestinal dysfunction, lethargy, easy fatigue, etc., without corresponding organic lesions. In the past, patients with this disease were transferred to various departments for treatment, and the names of the disease kept increasing while the treatment was often ineffective, and even aggravated the disease medically. In recent years, the Department of Pain Management has applied a comprehensive treatment method based on stellate ganglion block, supplemented by psychotherapy, to treat this disease, and obtained significant results. I. Etiology Psychosomatic factors such as overload of life events, dysregulation of the body to environmental conditions, etc.; endocrine dysfunction such as ovarian hypofunction, post-ovarian resection, etc. can also be causative factors. These causes lead to various clinical manifestations of the patient with a predominantly dysfunctional vegetative nerves, especially increased sympathetic tension. II. Clinical features The disease is mostly seen in menopausal and young women. The patient’s statements are extremely broad. Systemic statements include tiredness, fatigue, fever and night sweats, accounting for about 18%; neuromuscular statements include insomnia, dizziness, numbness, headache, stiffness of shoulder muscles, back pain, lumbago and weakness of lower limbs, accounting for about 36.3%; cardiovascular statements include palpitation, shortness of breath, chest tightness and swelling, accounting for about 22%. In addition, cold is also an important statement of the disease, mainly manifested as cold hands and feet, fear of cold environment, often prefer to use warmth or heating equipment, curled body sleep in winter, often prone to frostbite, etc.. In addition to the characteristics of many statements, the disease also has a temporal and spatial irregularity in the statements, such as the statement of pain in a certain area, but suddenly shifted to another unrelated area of pain or other discomfort. The aggravation of these symptoms is related to psychological and social factors, yet there is no corresponding organic lesion found during the examination. It is more likely to occur in menopausal and young women, and there may be psychological, social and endocrine factors in the history, and the psychological test may be inclined to somatization and hypochondriasis. 3. Indeterminate statements (symptoms) and signs do not match, and there is no positive development of various auxiliary examinations. Treatment 1. General treatment: regular life, appropriate participation in sports and cultural activities. Adapt or avoid bad living environment, maintain a good mental state. 2.Medication: vitamin B, vitamin E and placebo can be given, beta-blockers and other sympathetic nerve inhibitors can also be used. 3, stellate nerve block: 1% lidocaine 8~8mL, alternate block on both sides of the stellate ganglion, every 1~3 days block, usually 1~2 times there is a significant effect, 20~30 times can be expected to heal. The efficacy of the block can be judged according to the changes in appetite, sleep, vertigo, constipation, blood pressure, menstruation, cold, and postural regulation disorder items. The mechanism of stellate ganglion block for the treatment of this disease is mainly to release the excessive tension of sympathetic nervous system and regulate the disordered plant nervous function. 4, other: in particular, should pay attention to psychotherapy, other such as branch river therapy, acupuncture and TENS, Chinese medicine can be used in the treatment of this disease. Among the above methods, the effect of stellate nerve block is the most definite, but if it can be combined with other therapies for comprehensive treatment, the effect is better.
