Patient, female, 23 years old, employee. She was bitten by a silver-ringed snake on July 14, 2006, and was rushed to Anqing Municipal Hospital, where she suffered respiratory and cardiac arrest 5 minutes after admission. After cardiopulmonary resuscitation, his heartbeat was restored, but he did not breathe on his own and was ventilated with a ventilator. He was treated with “antivenom” for half a month, and neostigmine 0.5mg, Q8h, intramuscularly for 10 days, but the treatment of muscle weakness was not effective, and the symptoms of recurrent coma and respiratory failure did not improve significantly. He was admitted to the ICU of our emergency center on July 28, 2006, and was invited to our department for consultation. Examination: dazed, eyes open on call, both eyelids drooping, pupils equal in size and round, 4.5mm in diameter, direct light reflexes blunted, soft neck, nasal feeding, ventilator-assisted ventilation by tracheotomy, both lungs covered with wet rales, heart rate 125 beats/min, rhythmical, soft abdomen, diminished bowel sounds, liver and spleen not reached under the ribs, muscle strength of the limbs grade 0, low muscle tone, disappearance of tendon reflexes of both knees, pathological reflexes not elicited. The interosseous muscles of both hands were atrophied, the tongue was pale and fat, with little coating, and the pulse was fine. The patient had respiratory muscle paralysis and was ventilated with ventilator-assisted ventilation. The initial mode selected was synchronous gap command ventilation (SIMV) with tidal volume (VT) 350-500 mL; frequency (f) 12-18 times/min; pressure support (PSV) 10-15 cm H2O; and oxygen concentration (FiO2) 40%. After 17 pairs of acupuncture points (T12 and L5), i.e., one course of treatment (about 20 days), the patient’s condition gradually improved significantly, muscle strength of the limbs recovered significantly, muscle strength of both upper limbs was grade IV, muscle strength of both lower limbs was grade III, muscle tone was low, tendon reflex of both knees disappeared, respiratory muscle strength gradually recovered, ventilator-assisted ventilation mode was changed to spontaneous breathing mode (SPONT), PSV was 5~8 cmH2O, flow Trigger (Trigger) 3L/min,. After 10 days of treatment with acupuncture points mainly at the thoracic acupoints in the back, the ventilator-assisted ventilation was removed when the respiratory muscle strength returned to normal, and the patient was transferred to a general ward and continued acupuncture rehabilitation treatment for the sequelae. 10 days later, he was discharged. He was discharged from the hospital after 10 days. He has been living and working normally for two years. It acts on the presynaptic and postsynaptic membranes of motor nerve endings, mainly inhibiting acetylcholine receptors on the motor plate and blocking the neuromuscular junction to cause delayed paralysis, so people who are bitten by silver-ringed snakes tend to have respiratory muscle paralysis. In clinically critical patients, failure to restore spontaneous breathing and muscle tone after 3 to 14 days of artificially maintained respiration results in permanent damage and slower recovery. Silver-ringed snake toxin also impairs the conduction of the vegetative nervous system and affects the functional activities of various internal organs. According to Chinese medicine, this is a “wind poison” condition, in which the wind poison of snake venom invades the body and blocks the internal organs. In this case, the patient had two cardiac and respiratory arrests after the silver-ringed snake bite. It caused serious damage to the central nervous system and peripheral neuromuscular conduction function, which did not improve after half a month of anticholinesterase drug treatment. At this time, the patient’s heart, lungs, brain, kidneys and other important organs had been damaged to varying degrees, and his condition was critical. The names and distribution of the acupuncture points of the foot solar bladder meridian coincide with the internal organs innervated by the vegetative nerves of the spinal nerve segments. Although the elaboration is not as precise as modern anatomy and biology, the accuracy of the internal organs it describes is high. The patient had respiratory muscle paralysis, so we preferred to start treatment with the Lung Yu and its upper and lower Yu points. The yu point of the bladder meridian is located 1.5 inches below the spinous process of the thoracolumbar vertebrae and has spinal ganglion cell axons under the point. The selection of vitamin B for acupuncture injection can maintain the acupuncture effect for a longer period of time, play a role in excitation and nourishment of the nerves, and promote the recovery of the electrical activity function of the organs. The author also used this method to treat patients with subacute allergic myelitis and spinal cord cavitation (one case each) for a longer period of time, effectively controlling the progression and deterioration of the disease without the appearance of any side effects. These suggest to us that acupuncture point injection of the bladder meridian acupoints can not only treat the corresponding visceral disorders, but also promote the recovery of the function of the lesioned affected spinal cord.
