Gout prevention and treatment

  Gout (gout) is a recurrent inflammatory disease caused by increased purine biosynthesis, excessive uric acid production or poor uric acid excretion resulting in elevated uric acid in the blood and deposition of urate crystals in synovial membranes, bursae, cartilage and other tissues.  The disease is characterized by the presence of bifocal monohydrate urate crystals in joint fluid and gout stones. Its clinical features are: hyperuricemia and characteristic acute arthritis due to urate crystals and deposition, gout stones, interstitial nephritis, and in severe cases, joint deformity and dysfunction, often accompanied by uric acid urinary tract stones. With economic development and lifestyle changes, especially in coastal cities, its prevalence is gradually increasing.  In the clinical work, we found that the number of gout patients in our region is increasing year by year, and there is a trend of lowering the age of incidence. For this reason, our department organized several discussions, and jointly with the Department of Nephrology and the Department of Urology, we proposed the following treatment plan and research and development techniques.  1.Prevention of gout ①Alkalinize urine: prepare soda water of personal taste.  ②Education: make pamphlets for health education and scientific propaganda to patients.  ③Dietary intervention: annual health check-up for local residents, early intervention for those with elevated uric acid (less food with high uric acid: animal offal, seafood, less alcohol consumption) ④Drug prevention: drug intervention for patients with high uric acid or early gout.  ⑤ Drink more water.  2. Treatment of gout A. Drug therapy: ① Application of new drugs: search for the latest international drugs and analyze their efficacy.  ② Root cause treatment: xanthine oxidase replacement therapy.  ③ Combination of Chinese and Western medicine for the treatment of gout (may be acceptable from the patient’s point of view) ④ New drug development: development of drugs that are metabolized by the liver without any effect on kidney function ⑤ Small doses of estrogen (for postmenopausal women can be considered for application) B. Surgical treatment: The aim is to remove gout stones deposited in the joints through arthroscopy.
