Many patients come to the clinic and the most frequently asked question is how long they can live with glioma. This question is sometimes not easy to answer. In terms of grade, most grade 1 gliomas can be cured by complete resection, and if they are not completely removed due to bad growth site, most of them will recur and generally have longer survival. Grade 2 gliomas are relatively slow growing and most of them have a relatively good healing process, while grade 4 gliomas have the worst healing process and most of them have a short survival period. Gliomas above grade II usually persist or recur and require long-term treatment. Therefore, you need to actively cooperate with your health care provider, return to the hospital for regular follow-up visits, and follow your doctor’s recommendations for the next step of treatment. In terms of pathological type, oligodendroglioma is sensitive to radiotherapy and heals best. Diffuse astrocytoma is the next best, and glioblastoma has the worst healing. In terms of molecular pathology, most patients with IDH mutated types survived longer than those without mutations, and most patients with positive MGMT promoter methylation survived longer than those with negative ones. The survival of glioma patients is closely related to the extent of resection. Except for grade 1 gliomas, all grade 2 and above gliomas are diffusely growing, so it is very unlikely to achieve complete resection at the cellular level, so the more resection is done, the longer the survival is, and it is best to achieve extended resection. In order to prolong the survival of the patient, we have to do the maximum resection of the glioma in a safe way. The survival of glioma patients is difficult to find exact reference data. I have combined a set of internationally reported data to summarize the general survival predictions for patients with glioma as follows: grade II astrocytomas have an average life expectancy of 7-10 years, grade II oligodendrogliomas have an average life expectancy of greater than 10-15 years, grade III mesenchymal astrocytomas have an average life expectancy of 3.5 years, grade III oligodendrogliomas have an average life expectancy of greater than 10 years, and grade IV glial The average life expectancy for grade IV glioblastoma is 15 months, and the 2-year survival rate is about 27%. With the development of medicine, we have more and more treatments, and now most of the patients may exceed the above average survival, but some patients with low-grade glioma may relapse soon if not properly treated. The best treatment effect can be achieved.
