Proper understanding of nasal glucocorticoids

For children with allergic rhinitis and chronic sinusitis, doctors may recommend the use of nasal glucocorticoids, which are known by the trade names of Nesuna, Cozulan, and Mansi. Parents often talk about hormones and immediately associate them with full-moon faces, buffalo backs, failure to grow, etc. As pediatricians, it is also our important responsibility to use glucocorticoids appropriately. The use of glucocorticosteroids (commonly dexamethasone) for the sole purpose of reducing fever is never recommended. Rationale for choosing glucocorticoids Nasal glucocorticoids have powerful anti-inflammatory and anti-edema effects. In the early stages of inflammation, they reduce exudation, edema, capillary dilation, leukocyte infiltration and phagocytosis. In the later stage, it inhibits capillary and fibroblast proliferation, delays granulation, prevents adhesions and scar formation, and reduces sequelae. Children with allergic rhinitis often suffer from turbinate enlargement, resulting in nasal congestion and secondary sinusitis. Glucocorticoids can reduce the inflammatory response, relieve turbinate enlargement, and open up the drainage of the sinuses. Thus, they can be used as a treatment. Is nasal glucocorticosteroid safe Several nasal glucocorticosteroids are currently available 1. Plasma protein binding rate >90%. 2.Water-soluble preparation, low bioavailability <1%. 3. Topical use, small dose, low systemic activity, low inhibitory effect on hypothalamic-pituitary-adrenal axis. Therefore, it is safer to use locally. Contraindications to glucocorticoids There are severe psychiatric disorders, epilepsy, active peptic ulcer, corneal ulcer, hyperadrenocorticism, severe hypertension, diabetes mellitus, infections that cannot be controlled by antibacterial drugs such as chickenpox and mycobacterial infections. When indications and contraindications coexist, a comprehensive analysis should be made, the pros and cons weighed, and a careful decision made. How to use nasal glucocorticosteroids Safer does not mean that the drug can be used arbitrarily. It should be used in a regular and reasonable manner, according to the course of treatment, pharmacology, and according to the condition. For children, safety is our main concern. Therefore, the medication should be regulated under the guidance of doctors, which will help to relieve the disease as soon as possible, reduce the use of other comprehensive treatment drugs and shorten the course of treatment. Recommendation 1.Use the medication strictly under the doctor's guidance, including the dose, the method of use, etc. 2. Do not stop the medication at will. Non-standardized treatment, on the contrary, will lead to longer medication time, increased dose and poor effect.
