Dietary therapy for gout

  (i) Maintain ideal body weight Epidemiological survey found that serum urate level is positively correlated with obesity degree, body surface area and body mass index. (Some scholars suggest that the daily purine intake should be below 100-150mg, especially the intake of purine-rich food should be limited. Since protein has a special role in the body, excessive protein intake can also increase endogenous uric acid, so it should also be appropriately limited.  (3) Encourage the consumption of alkaline foods Foods containing more sodium, potassium, calcium, magnesium and other elements are oxidized in the body to produce alkaline oxides, such as vegetables, Maling potatoes, sweet potatoes, milk, etc., which are physiologically called alkaline foods. Fruits, such as citrus, are also alkaline foods because they are rich in alkaline potassium after metabolism in the body. Increasing the intake of alkaline food can reduce the acidity of serum and uric acid, and even make the urine alkaline, thus increasing the solubility of uric acid in the urine (d) Ensure sufficient urine volume If the patient has normal cardiopulmonary function, the urine volume should be maintained at about 2000ml/d to promote uric acid excretion. Therefore, the total daily fluid intake of the patient should be 2500-3000ml, and drinks such as plain water, tea, mineral water, soft drinks and fruit juices are appropriate. However, strong tea, coffee, cocoa and other beverages have stimulating effects on the plant nervous system and may cause gout attacks, so they should be avoided.  Methods (a) Limit total calories Total calories are calculated according to the patient’s ideal body weight at rest, and usually do not exceed 105-126kJ (25-30kcal)/kg per day. clinical experience shows that if an adult patient is obese above a certain degree (30%-50% overweight), the total calories exceed 6300kJ per day, and often cannot make weight loss. The following methods can be used to limit total calories and reduce body weight for reference.  1. Total calories for patients who are 30% to 50% overweight or above should start at 6300kJ/d and be divided into three meals. After one month, change it to 5460kJ/d; or reduce calories by 2310~4620kJ/d on the basis of the original diet, with the aim of losing 0.5~1.0kg of body weight per week.  2, overweight or mildly obese people total calories to 6300kJ/d to start, divided into three meals supply; or in the original diet based on the reduction of calories 525 ~ 1050kJ/d, in order to achieve the purpose of monthly weight loss.5 ~ 1.0kg.  (B) the distribution of the three major nutrients Under the premise of limiting the total calories, the distribution principles of the three major nutrients are: high carbohydrates, medium amount of protein and low fat.  1.Carbohydrates include vegetables and fruits, which should account for 65% to 70% of the total calories. This is also in line with the national diet, so that it can reduce fat decomposition to produce ketone bodies, which is conducive to uric acid salt excretion. But should try to eat less cane sugar or beet sugar.  2, protein protein should account for 11% to 15% of total calories, usually 0.57 to 1.0g/kg per day. mainly use milk, cheese, skim milk powder and egg protein part. Because they are high-quality proteins rich in essential amino acids, which can provide the needs of tissue metabolism constantly updated, and contain very little purine, they have little adverse effect on gout patients. However, yogurt is not good for gout patients because it contains more lactic acid.  3. The rest of the total calories from fat is supplemented by lipids, usually 40-50 g/d. Since the oxidation of fat produces about two times more calories than carbohydrates or protein, it should undoubtedly be limited in order to reduce the patient’s body weight.  Precautions (a) Avoid drinking alcohol The main component of alcohol is ethanol, which can induce glycogen isogenesis disorder and lead to the accumulation of lactic acid and ketone bodies in the body. The β-hydroxybutyric acid in lactate and ketone bodies competitively inhibits uric acid excretion.  (2) Individualized purine restriction The restriction of purine intake should be treated separately according to the severity of the patient’s disease, the stage of the disease, comorbidities and the application of uric acid-lowering drugs, i.e., in accordance with the patient’s individual situation.  (c) Pay attention to the cooking method of food A reasonable cooking method can reduce the amount of dextrine contained in food, such as cooking meat first and discarding the soup before cooking. In addition, foods and seasonings such as chili, curry, pepper, pepper, mustard and ginger can excite the plant nerves and induce acute attacks of gout, so they should be avoided as much as possible.
