How to prevent and control diarrhea in summer and autumn?

Acute gastroenteritis is a common and frequent disease in summer and autumn, mostly due to bacterial and viral infections. The main manifestations are upper gastrointestinal symptoms and varying degrees of diarrhea and abdominal discomfort, followed by electrolyte disturbances and fluid loss. The disease belongs to the category of “vomiting, abdominal pain and diarrhea” in Chinese medicine.
  I. Etiology of diarrhea
It is mainly caused by acute inflammation of the mucosa of the gastrointestinal tract due to the consumption of food containing pathogenic bacteria and their toxins, or improper diet. The incidence is higher in summer and autumn, and the general incubation period is 12 to 36 hours. Salmonella spp. is the main pathogenic bacteria causing acute gastroenteritis.
Chinese medicine believes that the body is deficient, overeating, cold and cold drinking, as well as taking toxic drugs are important causes.
The first major cause is unhygienic consumption of food, or eating poisonous food, such as sprouted potatoes, poisonous mushrooms, white fruits, etc., which is generally known as food poisoning.
The second cause is the consumption of large amounts of alcohol, or taking certain drugs that directly irritate the gastric mucosa.
The third is the accidental consumption of drugs and poisons that are damaging to the stomach and intestines, such as organophosphorus pesticides, heavy metals, digitalis, cholinergic drugs, laxatives, etc.
The fourth is hot and cold stimulation or overeating, which is more closely related to the summer. When the climate is hot, people are willing to drink some cold drinks or eat some cold food, there to take away the feeling of heat, but it can make the gastrointestinal tract dysfunction. In addition, overeating, eating a large amount of food or drinking too much water at a time can also cause gastrointestinal tract dysfunction. The volume of the stomach suddenly increases during overeating and the gastrointestinal mucosa is damaged, in this case, acute gastroenteritis caused by holidays is more common.
The fifth is stress, mainly refers to the body in a state of stress in the case of serious infection, major trauma, major surgery, shock, mental trauma, etc., the gastric mucosa due to vascular spasm and ischemia, insufficient mucus secretion, barrier destruction and erosion, bleeding, and even ulcers. Biological factors are the most common among the above etiologies, and the majority of acute diarrhea is caused by pathogenic microorganisms.
  Second, the classification of diarrhea
  1, infectious diarrhea
  (1) bacterial infection: acute bacterial dysentery, cholera and para-cholera, enteric typhoid, Staphylococcus aureus enteritis.
  (2) viral diarrhea: rotavirus, adenovirus, norovirus.
  (3) fungal diarrhea: Candida albicans, Aspergillus, yeast, etc.
  (4) parasitic infections: amoebic dysentery, schistosomiasis enteropathy, cryptosporidiosis.
  (5) Food poisoning: Salmonella, pathogenic Escherichia coli, Staphylococcus aureus, Aspergillus, Salmonella, Clostridium botulinum and other contaminated food.
  (6) Toxic substances and drugs: puffer fish, fish bile, animal liver, poisonous mushrooms, sprouted potatoes, organophosphorus pesticides, digitalis, cholinergic drugs, laxatives, etc.
  2.Non-infectious diarrhea
  (1) intestinal diseases: acute hemorrhagic necrotizing small intestine, inflammatory bowel disease, intestinal tumors, irritable bowel syndrome, diverticula, etc.
  (2) malabsorption syndrome: pancreatic enzyme deficiency, bile salt deficiency, post-gastrectomy, lactase deficiency, intestinal flora dysbiosis, partial small bowel resection, etc.
  (3) Endocrine tumors: gastrinoma, vasoactive intestinal peptide tumor, carcinoid tumor.
  (4) Systemic diseases: uremia, diabetes mellitus, thyroid crisis, allergic purpura.
  3.Common bacteria that cause gastroenteritis
  (1) Escherichia coli: is a very important causative agent of acute gastroenteritis, which can cause disease ranging from mild diarrhea, self-limiting diarrhea to severe diarrhea with high mortality. Enterohemorrhagic E. coli is a serotype bacterium that causes severe diarrhea and can cause fulminant epidemics of hemorrhagic enteritis. Uncooked hamburgers and unpasteurized fruit juices from fast food restaurants can carry E. hemorrhagicus. The bacterium is present in cattle, but does not make them sick and can be transmitted to humans through contaminated beef.
  (2) Salmonella: is a group of bacilli parasitic in the intestinal tract of humans and animals, Salmonella spp. bacteria have many types, about 400 kinds, such as Salmonella typhi, Salmonella enteritidis, etc., some animal as host, can infect both animals and people, can cause human food poisoning or sepsis, such as Salmonella cholerae, duck Salmonella, Salmonella typhimurium. Salmonella spp. is the main pathogen causing acute gastroenteritis, including Salmonella typhimurium, Salmonella enterica, Salmonella cholerae, Salmonella chicken, Salmonella duck are more common.
Salmonella must enter through the mouth in sufficient quantities to overcome the body’s protective barrier (the role of gastric acid, the normal intestinal flora) and localize directly in the small intestine in order to cause disease. Salmonella has a strong endotoxin, individual can produce enterotoxin. Many Salmonella are zoonotic pathogens, and humans can become ill from eating meat, milk, eggs, dried cheese or cream from sick or germ-bearing animals, as well as from snapping fish kept as pets and food contaminated with urine and stool from sick rats. Water sources contaminated by feces often cause outbreaks, transmission is mainly fecal-oral transmission, children under 5 years of age account for 25% of all cases.
  (3) Vibrio: diarrhea caused by Vibrio cholerae and Vibrio parahaemolyticus is more common in developing countries. Vibrio generally live in water, salinity, eating raw or uncooked seafood (such as jellyfish, shrimp, shellfish, etc.) or salt cured products, containers or plates raw and cooked contaminated with Vibrio, can cause food poisoning or cholera outbreaks. After 24 to 48 hours of eating, patients will have low fever, chills, headache and vomiting, followed by diarrhea and abdominal pain, and watery or bloody stools; mild cases can heal spontaneously, and severe cases should be treated aggressively. Cholera is a typical enterotoxic disease, with typical symptoms of persistent large amounts of rice swill-like stools, massive fluid and sodium loss leading to dehydration and shock.
  (4) Campylobacter: widely distributed in the intestinal tract of poultry and wild birds, can cause gastroenteritis and sepsis in humans. The human pathogenic Campylobacter jejuni, Campylobacter coli and other 13 species. The bacteria often cause disease through contaminated diet (mainly chicken and duck meat), milk, water, etc. Symptoms include crampy abdominal pain, bloody or jam-like stools, headache, fever.
  (5) Staphylococcus aureus: a common pathogen present in contaminated high-protein foods such as salad dressing, ham, poultry, potatoes, cakes, etc.
Patients will vomit and have diarrhea 1 to 2 hours after consuming contaminated food, and get better after 6 to 10 hours. Although the symptoms occur briefly, they are very serious and even require hospitalization.
  (6) Clostridium perfringens and Bacillus cereus: due to the production of toxins, can also lead to the occurrence of short-term diarrhea, Clostridium perfringens is more common than Bacillus cereus in uncooked or reheated meat, poultry and broth, gravy. Bacillus cereus, on the other hand, is commonly found on steamers in cafeterias.
  4.Symptoms of diarrhea
The main clinical manifestations are rapid onset, abdominal distension, pain in the upper abdomen or around the umbilicus, nausea, vomiting, diarrhea, etc. Serious cases may lead to fever, dehydration, electrolyte disorders, shock, etc. Patients mostly show nausea and vomiting first, followed by diarrhea, ranging from 3 to 5 times a day or even dozens of times a day, with watery stools, thin and soft stools, jelly-like stools, dark yellow or greenish or bloody stools, etc. Some stools have an unpleasant odor. Patients may have abdominal cramps, fever, generalized aches and pains, depression, and loss of appetite. In case of inflammation of the posterior segment of the colon or dysentery, there is often urgency. Enteritis caused by viruses, bacteria, fungi and parasites, etc., see their corresponding symptoms.
  5.Diarrhea diagnosis
On physical examination there is pressure pain in the upper abdomen and around the umbilicus or in the left lower abdomen, and the bowel sounds are mostly hyperactive.
In bacterial infections, there can be a small amount of mucus and red and white blood cells in the stool microscopy, and sometimes phagocytosis can be seen. The corresponding pathogenic bacteria can be cultured in the vomit and diarrhea.
The blood count is normal or elevated, and the white blood cell count and neutrophil percentage are elevated in bacterial infections.
6, the prevention of acute gastroenteritis
  (1) Keep the “disease from the mouth” off: pay attention to diet, water and food hygiene. Pay attention to personal hygiene, wash your hands before and after meals, wash and scald raw fruits and vegetables, peel and eat fruits, do not drink raw water, eat more garlic and vinegar when eating out, dispose of garbage and feces in a harmless manner; sanitary food utensils, store raw and cooked food separately, separate raw and cooked plates; strengthen the fight against flies and mosquitoes; strengthen the management of toxic drugs and pesticides.
  (2) Control bacterial reproduction: The main measures are refrigeration and freezing. Temperature control at 2 ℃ ~ 8 ℃, can inhibit the reproduction of most bacteria. Cooked food in the refrigeration to avoid light, oxygen cut off, no repeat contamination, refrigeration is better.
  (3) high-temperature sterilization: food before consumption for high-temperature sterilization is a reliable method, the effect of the temperature, heating time, the type of bacteria, the amount of contamination and the processed food traits and other factors, general meat food boiling 15 minutes to kill common pathogenic bacteria.
  (4) vegetables should be properly stored: keep fresh, prevent decay, do not eat rotten vegetables. Leftover cooked vegetables should not be stored at room temperature for a long time before consumption. Food should be thoroughly washed, conditioning and storage places, utensils, containers should be kept clean.
  (5) The amount of nitrate and nitrite in meat products should not be excessive: always cook seafood, always wash hands before meals, and always disinfect dishes. Do not use bitter well water for cooking porridge, especially do not store it overnight.
  (6) Do not eat a lot of freshly pickled vegetables: pickled vegetables should be more salt, at least to 15 days before eating. Pickled vegetables when using fresh vegetables.
  (7) evil wind, avoid the sometimes: pay attention to the appropriate cold temperature, avoid evil, infectious disease epidemic season less to go to public places, reduce the opportunity to contact with patients. Avoid contact with chemical poisonous gas, industrial waste gas, pesticides and mountain miasma.
  (8) Maintenance of vital energy: eat less or do not directly consume frozen food and drinks to prevent damage to Yang Qi and cold and dampness in the spleen and stomach. In summer, sweating can drink warm water, adding the right amount of salt, sugar, sour plum crystal. Do not eat and sleep in the open, do not blow the fan or lower the temperature of the air conditioner arbitrarily. Do not stay up late, do not overeat, overeat, overfed, overworked, do not eat spicy, sticky and fishy fried food.
  (9) Emotional and moral harmony: to make the five will harmonious, no excess and less than. If the mood is uncomfortable, anxious and worried, depressed and nervous, can be five will fire, resulting in internal heat and poison, once the regulating carelessness, feel the time evil epidemic poison or diet carelessness, then the internal and external combination of evil, the onset of the disease rapidly.
  (10) Conformity to the four seasons of health: dress appropriately and comfortably, and increase and decrease clothing in accordance with the natural climate. Spring and summer nourish Yang, autumn and winter nourish Yin, always pay attention not to hurt Yang Qi, protect Yin Qi, sleep at night and get up early, walk widely in the court, be hair and slow shape, so that the will is born, born but do not kill, give but do not take, shade to avoid the heat, movement to avoid cold.
  (11) Timely rehabilitation and treatment of chronic diseases: such as diabetes, anemia, chronic hepatitis, chronic gastritis, etc., to prevent deficiency due to illness and evil due to deficiency.
  (12) Treating the untreated diseases: there are two meanings, one is to prevent the untreated diseases before they occur, and the other is to prevent the changed diseases. Chinese medicine believes that there are both internal and external causes of diseases, so the treatment of the untreated diseases should focus on both internal and external causes, on the one hand, to strengthen the righteousness, on the other hand, to protect against external evil, how to combine them? How to combine the two? The “Post-Jin Pao Jue” says: “To know that those who are prone to wind as a disease, the surface Qi is weak; those who are prone to cold as a disease, the Yang Qi is weak; those who are prone to heat as a disease, the Yin Qi is weakened; those who are prone to food injury, the spleen and stomach will be deficient; those who are prone to labor injury, the middle Qi will be lost.” This means that diseases are mostly caused by the combination of internal and external evils.
Therefore, those who have a weak spleen and stomach should pay attention to warming and tonifying; those who have a strong internal heat should pay attention to nourishing Yin and clearing heat; those who easily feel wind and cold should pay attention to solidifying the surface of the body; those who have a weak digestion of the spleen and stomach should pay attention to not injuring food; and those who have no disease should make up for the deficiencies before they occur.
Once the disease has occurred, we should try to alleviate the disease, prevent the disease from spreading and change, and first secure the place where the evil has not been suffered, for example, if you are already overly angry, do not eat again, why? The liver is responsible for regulating the emotions and helping the spleen and stomach to transport and transform.
7, acute gastroenteritis home Chinese medicine treatment
The following are a few of the emergency treatment methods.
  (1) Replenish fluids and electrolytes. Once you have gastroenteritis, you need to drink more fluids to prevent deficiency, you can choose plain water and other liquids, such as apple juice, grape juice, broth, when replenishing water, do not swallow it all in one gulp, so as not to trigger vomiting again.

8, the acute gastroenteritis diet adjustment
The actual fact is that the actual evil is the cause of the disease, but it varies from person to person, male, female, old and young physique, conditioning should also be treated separately. The principle of the deficiency is to make up for it, the cold is warm, the food is eliminated, the wet heat is clear, neither bitter cold seepage too much, fear of injury to the vital energy, but also should not be too early solid astringent astringent, prevent stagnation to stay evil.
  (1) A light diet. Usually you can start eating within a few hours to a day after the diarrhea or vomiting has subsided, but you need to take it slowly because the injured stomach is still weak. Start with easily digestible foods, such as millet porridge, cereal, thick rice noodle soup, light juice pudding, cookies, etc. After the bowel movement decreases during the remission period, you can eat a liquid diet with less oil such as broth, soy milk, egg white soup and vegetable juice. Later, gradually eat a light, less oily, less residue of semi-liquid diet, and then transition to a normal diet.
  (2) Avoid high-fiber, spicy, acidic, greasy, cold, raw and cold, dairy foods to avoid stimulation of the stomach and intestines and the production of damp heat.
  9.Commonly used dietary treatment formula

10.Summer herbs can be used for medicine and food
  (1) Garlic: also known as Hu Garlic, was introduced from the West in ancient times. It is pungent and warm in nature, and can be used for stagnation, swelling, detoxification and insecticide. Whenever there is an epidemic, folk use garlic as an antiseptic and detoxifier. It is said that in ancient times, a general commanded his soldiers to attack a city, but there was an epidemic in the city and the troops were in danger of being wiped out, so the general ordered his soldiers to eat a lot of garlic and finally won the victory. In World War II, the British army bought thousands of tons of garlic to make up for the lack of antimicrobial agents.
Modern research shows that garlic contains volatile substances, such as allicin (which has been made into tablets and injections), and has an inhibitory effect on staphylococcus, pneumococcus, tuberculosis bacillus, bacillus dysenteriae, and even certain drug-resistant bacteria and fungi. Garlic has a complex composition and has a variety of biological activities, which can improve appetite, promote gastric juice secretion, and is useful for preventing high blood pressure, hypertension and hyperglycemia, as well as anti-cancer effects. Commonly used food methods are sweet and sour garlic, garlic puree eggplant, garlic pat cucumber, etc.
  (2) Coix seed: Coix seed has more than ten kinds of names such as Coix rice, rice kernel, pearl rice, known as the “world’s first rice”, “the world’s king of grass plants”, Coix seed occupies an important position in medicine, food therapy, medicinal cuisine, its taste is sweet, light, slightly cold, into the spleen, kidney, lung meridian, its health. It is sweet, light and slightly cold in nature and enters the spleen, kidney and lung meridians. The “Compendium of Materia Medica” says: “Job’s tear, yin and yang medicine, can strengthen the spleen and benefit the stomach. Coix seeds contain high protein, riboflavin, vitamin E, potassium and a variety of trace elements, pharmacological research shows that there are antipyretic and analgesic, antihypertensive and hydrophobic, stimulating respiratory effects. For patients with diarrhea who have a weak spleen and stomach, use it to cook porridge with adzuki beans, sorghum rice and corn.
  (3) Amaranth: also known as horsetail, horsetail, longevity, anole, tastes sour, cold, contains protein, fat, sugar, crude fiber and calcium, phosphorus, iron and other nutrients, also contains a lot of norepinephrine and a lot of potassium salt. Norepinephrine can promote insulin secretion, regulate the body’s sugar metabolism, with lower blood glucose concentration, maintain the role of blood sugar stability. Potassium salts can lower blood pressure and slow down the heart rate, which has a protective effect on the heart. Amaranthus also contains a lot of vitamin E, vitamin C, carotene and glutathione, alpha-linolenic acid and other anti-aging and lipid-lowering effective ingredients.
It can nourish the epithelial tissues and promote the healing of ulcers in the mouth, stomach and duodenum. It is a good remedy for dysentery and enteritis. The decoction can also be used to treat acute diarrhea with scutellaria, dandelion, mullein and patchouli decoction.
Bitter melon porridge: 100 grams of bitter melon, 60 grams of round-grained rice, 15 grams of amaranth, 100 grams of rock sugar. How to use: Wash the bitter melon and remove the flesh, cut it into small dice, wash and chop the amaranth and set aside. (4) Honeysuckle: It is the flower bud of the vine Lonicera japonica, sweet in taste, cold in nature, and belongs to the lung, stomach and large intestine meridians. It is effective in dispersing wind-heat, clearing heat and detoxifying. It has antibacterial, antiviral, antipyretic, anti-inflammatory, hepatoprotective, hemostatic, antioxidant and immunomodulatory effects.
In vitro experiments have shown that honeysuckle has certain inhibitory effects on a variety of pathogenic bacteria, including Staphylococcus aureus, Streptococcus hemolyticus, Streptococcus pneumoniae, Diplococcus meningitidis, as well as S. typhi, S. paratyphi, S. dysenteriae, Vibrio cholerae, Pseudomonas aeruginosa, Escherichia coli and Mycobacterium tuberculosis. If combined with forsythia, the antibacterial range can also be complementary. Combined with penicillin, it can strengthen the antibacterial effect of penicillin on drug-resistant Staphylococcus aureus.
Antiviral effect, aqueous decoction has inhibitory effect on influenza virus, orphan virus and herpes virus. Chrysanthemum aqueous decoction has a very obvious inhibitory effect on coxsackievirus and echovirus extracellularly. The antibacterial and antiviral active ingredients are generally considered to be chlorogenic acid compounds.
①Anti-inflammatory: Honeysuckle has obvious anti-exudative, anti-proliferative and antipyretic effects, and its aqueous decoction, oral solution and injection have different degrees of antipyretic effects.
  ②Immunomodulatory effect.
  ③Induced production of interferon.
  ④Anti-bacterial endotoxin effect.
  (5) Elimination of oxygen free radicals.
  (5) fishy grass: fishy grass is also known as stinky lingdan, dog’s ear, fishy grass contains volatile oil, decanoyl acetaldehyde, chlorogenic acid, quercetin, quercetin, rutin, fishyllin, etc..
As early as more than two thousand years ago in China, the fishy grass as a wild vegetable with food. It is said that during the Spring and Autumn Period, when King Goujian of Yue was lying on his back and refining his will, he led the people to choose Houttuynia cordata (fishy grass) and eat it to feed their hunger and survive the famine. For a long time, it has been playing the dual role of medicine and food, playing a role in health care and disease prevention and treatment for the people. In various places (especially in the southwest of China), wild or home-grown fishy grass has become the “big road wild vegetables” on the table of the public.
It is pungent and cold in nature, and can clear heat and detoxify, reduce swelling and heal sores, diuretic and remove dampness, and strengthen the stomach and eliminate food. It is used to treat actual heat, heat and poison, dampness and fever for lung canker, hemorrhoids, blood in the stool, sinusitis, and heat in the spleen and stomach. Pharmacological research shows that fishy grass has antibacterial effects on various pathogenic bacteria (such as Escherichia coli, dysentery bacillus, etc.), cocci, influenza virus, etc. It has significant antibacterial and anti-inflammatory effects. Fishy grass has anti-allergic and anti-cancer effects. Fishy grass vegetation can enhance the phagocytic ability of white blood cells, and has diuretic and detoxification effects.
Consumed as a vegetable, fishy herb is good for preventing skin boil infections, bacterial dysentery, and heat stroke, which are common in summer. There are several common ways to eat: first, the underground stems of fishy grass to remove the nodes of the hairy roots, clean and cut into small sections of 2 to 3 centimeters (can also be added to the young leaves), put vinegar, soy sauce, chili powder, monosodium glutamate and other condiments cold raw, crisp and refreshing, fresh and fishy taste; second, the underground stems together with the young stems and leaves boiled soup, fried, fried or stewed, fragrant and pleasant, the mouth is pleasant to melt, slightly fishy taste; third is pickled and processed into pickles to eat The third is pickled and processed into pickled vegetables for consumption, sour and crisp, appetizing.
  (6) radish: radish has white, red, green, purple different varieties, native to temperate regions of Asia. The medicinal use of red-skinned white spicy radish is the best, its alias Lycopodium. Records about radish have been written in the Poetic Edda more than 2500 years ago. Radish is a dual-use plant for medicine and food, and is an extremely common root vegetable, which was said by Li Shizhen, a great medical scientist of the Ming Dynasty, to be “the most beneficial of vegetables. The “Materia Medica Jing Shu” says: “Lycopodium root down the gas and eliminates grain, remove phlegm, fat healthy people can warm the middle, fill the shortage, broaden the chest and diaphragm, facilitate urination and defecation, resolve phlegm and eliminate the guide, cooked also; to stop thirst, control the face poison, the wind, remove the evil heat, cure lung impotence vomiting blood, lung heat phlegm cough dysentery, raw food also.”
It is suitable for indigestion, bloating, cough with phlegm, chest congestion, vomiting, constipation, dry throat and thirst, gas poisoning and other diseases. It is a good vegetable and medicine that people like to receive.
For the treatment of dysentery after heavy congestion, Lycopodium officinale five dollars, white peony three dollars, rhubarb one dollar, mucuna pruriens five minutes, decoction with water.
For the treatment of red dysentery, carrot stick three dollars, Shen Qu two dollars, hawthorn three dollars, sugar two dollars, decoction in water.
