A hospital received such a case: The patient was a 36-year-old woman who had been accustomed to hip dysplasia in her left leg for a long time since she was a child. Recently, she felt particularly worried because her child’s school exams were not going well, and in turn, she felt that the leg problem had worsened and that it was difficult to walk. She went to the orthopedic department of the hospital to see her leg, but the doctor thought that she had missed the time for treatment at her age and that the treatment would not be effective. This added to her psychological burden and even made her feel that there was no future for her and she lost confidence and interest in life. The doctor diagnosed that the woman was actually suffering from occult depression. “Its prominent manifestation is a sudden intolerance to stress to which one has become accustomed.” For 36 years, the patient had been married, working and having children as normal, and none of the leg problems had gotten in the way of life. Now suddenly she finds it unbearable, not because external pressure has overwhelmed her, but because there is something wrong with her inner tolerance. The patient is depressed, feels hopeless, and has no interest in doing anything. …… patients show some of the core symptoms of depression. Why is it called “insidious depression”? The doctor explained that because the patient has physical symptoms as the main, prominent manifestation, which conceals the depressive mood. According to the doctor, occult depression is a kind of atypical depression, mainly manifested by recurrent or persistent various somatic discomfort and vegetative symptoms, such as headache, dizziness, palpitations, chest tightness, shortness of breath, numbness of the limbs and nausea, vomiting and other symptoms, patients often go to internal medicine, neurology, Chinese medicine, but often can not find out what the problem is, the untreated physical pain or discomfort makes patients to their own body changes overly sensitive and concerned. They are not aware of their mood changes or believe that somatic diseases cause depressed mood. Occult depression is easily overlooked and misdiagnosed. So, how should one distinguish occult depression from general physical illness? The doctor gives two criteria to determine this. Patients present with physical discomfort but no corresponding organic disease or organ damage is detected, i.e., physical organic disease should be ruled out first. There are also depressive symptoms, which are manifested as: unhappiness, lack of interest, and lack of energy, which are the three core symptoms of depression. If the above criteria are met, then the patient is probably suffering from insidious depression and should be seen by a psychiatrist as early as possible to avoid delaying the best treatment. Depressed patients do not always have suicidal thoughts, only a small percentage of them want to die, and many others are very afraid of death. Some people with mild depression can also do things like ordinary people, with no apparent problems, but they have an impaired ability to experience pleasure inside, no pleasure, no confidence and are miserable. ”Currently, occult depression is relatively common in psychiatric clinics, and physical symptoms are significantly reduced with antidepressant treatment.” A patient with depression with diabetes was once seen, an elderly woman in her 50s. Her back blew cold and she felt back pain, so she went to the hospital for examination. After she was hospitalized, the pain in her back became worse and worse, and her whole body was cold and hot, which she described as “hot and numb”. Later, she found it difficult to walk and sit in a wheelchair. She wanted to commit suicide, but she didn’t even have the strength to hit the wall. The patient did not think she was suffering from depression, but according to the test results, she did not have a particularly serious medical condition. After taking antidepressants, her condition began to improve, she slowly tolerated physical pain, could get out of bed, her mood gradually recovered, and later even her diabetes improved.
