Example of a clinical case of Si Miao Wan

Si Miao Wan is composed of Atractylodes cantonensis, Phellodendron cantonensis, Job’s tears, and Ox knee, and has good efficacy in treating multiple diseases caused by damp-heat infusion. The author used this formula to treat a variety of diseases in the clinic from 2009 to 2011, and achieved good results, as summarized here. 1. Internal medicine Wang, male, 12 years old, was first diagnosed in November 2010. He had recurrent purple-red bleeding spots on both lower legs for one month. The bleeding spots on the extensor surfaces of both lower legs appeared after a cold one month ago, and were deep red to purplish in color, accompanied by abdominal pain, red urine, and swelling and pain in both knees. He was diagnosed with allergic purpura and was treated with Radix Polygoni, Pansentin, Paracetamol, Rutin and Scopolamine, and all symptoms improved. The strain of physical education led to the reappearance of purpura in the lower leg, accompanied by mild abdominal pain and joint pain, with red tongue, yellow coating and slippery pulse. The diagnosis was grape disease, with evidence of damp-heat infusion and blood flow. The treatment is to clear heat and dampness, invigorate blood and open the channels. The medicine used Cang Zhu, Huang Bai, Dan Pi, Xi Gen 10g each, Chuan Niu Kne, Ze Xie, Red Shao, size thistle 15g each, Job’s Tear, Bai Mao Root, Tu Fu Ling, Slippery Rock 30g each, 1 dose a day, after 6 doses, purpura gradually receded, no new rise. The effect does not change the formula, and then into 12 doses, the disease is cured. Follow up six months without recurrence. Press: Allergic purpura is a more common microvascular allergic hemorrhagic disease. According to the clinical manifestations, it is divided into simple type, abdominal type, joint type, renal type and mixed type. In Chinese medicine, it belongs to the category of blood evidence, and according to different manifestations, it is also called purple spot, grape disease, blood in urine, epistaxis, etc. In the early stage of the disease, the pathogenesis is mostly due to the downward injection of dampness and heat, forcing blood to move and stagnation of meridians. The treatment is to clear damp-heat, invigorate blood and open the channels. In this formula, Si Miao Wan, Ze Xie, Tu Fu Ling, Slippery Rock and Bai Mao Root clear heat and dampness, while Dan Pi, Xie Root, Red Peony and Large and Small Thistle clear heat and cool the blood to stop bleeding. The symptoms disappeared when the heat was removed and the dampness was cleared and the blood stopped. 2. Dermatology Zhao, female, 23 years old, was first diagnosed in August 2010. She had acne on her face for six months. The acne is scattered on the forehead, cheeks and around the mouth and lips, partly with pus heads, with purplish red acne, oily face, bitter mouth and bad breath, constipated stool, yellow urine, red tongue with yellow greasy coating and slippery pulse. The diagnosis is acne. The evidence belongs to heat in the lung and stomach and dampness and heat within the body. The medicine is based on 15g each of Atractylodes macrocephala, Cyperus rotundus, Radix Platycodon, Scutellaria baicalensis, Gardenia jasminoides, 10g each of Radix rhubarb, 10g each of Glycyrrhiza glabra, 30g each of Job’s Tears, Psidium guajava, Poria cocos, 15g each of Radix et Rhizoma spp. The above formula was supplemented with 15g each of Zhe Bei and Gong Ying, 10g each of Soapberry, and 6g of Rhubarb, 1 dose every other day. The rest of the formula was continued with 1 dose every other day and 6 more doses were taken, and the acne was completely gone, leaving dark brown skin. Without changing the formula, 1 dose every 2 days, the treatment lasted for 2 months and the skin on the face was normal. The most direct factor is that the pores are blocked, and the oil in the hair follicles cannot be discharged, and the more it accumulates, the more it forms a small pimple. It is a good idea to have a good look at the face and chest, back and shoulders of young people. The patient is the result of the damp heat of the lung and stomach with the head and face. The formula in the four magic pills, Tu Fu Ling, bitter ginseng to clear damp heat, Scutellaria baicalensis, Gardenia jasminoides to clear heat and detoxify, cooked rhubarb, the whole Gua Pou to laxative heat, raw earth to cool the blood and detoxify, thorny mustard, anti-feng counter-supportive, with the orchid to enter the lung meridian to promote clearing and dispersing, and prevent the cold too much, and then add Zhe Bei, soapberry to soften the hardness and disperse the knots, to clear the heat and dampness to go to the stool through the turbidity, the disease disappeared. 3. Stomatology Shan, male, 48 years old, was first diagnosed in July 2011. He has had recurrent oral ulcers for 10 years, which have been aggravated for 1 week. The ulcers are mostly on the buccal mucosa, the edge of the tongue and the inner side of the lips. The pain is severe and affects eating and speaking, the stool is constipated, the urine is yellow and burning, the tongue is red with a thick yellow coating, and the pulse is stringent and slippery. He has a history of alcoholic beverages and meat and seafood. He was diagnosed with mouth sores, which are caused by damp-heat in the spleen and stomach. The treatment is to clear damp-heat, drain turbidity and detoxify. The medicine used is 15g each of Phellodendron Bark, Atractylodes Macrocephalae, Bacopa monniera and Radix et Rhizoma Glycyrrhizae, 10g each of Radix Angelicae Sinensis, Radix Rehmanniae Sinensis and Rhizoma Recutitae, 20g each of Radix Achyranthes Bidentatae and Rhizoma Polygonati, 30g each of Job’s Tear, Radix Paeoniae Alba, Radix Bupleurum Multiflorum and Poria Cocos, and 6g of Bamboo Leaf. After taking 5 more doses, the second stool was relieved and the mouth ulcer had mostly healed. The effect is not to change the prescription, and 1 dose was prescribed every other day, and 3 more doses were taken to heal the oral ulcer. Remove bamboo leaf and white fungus root, reduce rhubarb to 6g, add Astragalus membranaceus 15g, instruct him to take one dose every other day, and then take 3 doses to heal. Press: Recurrent oral ulcers are the most common oral mucosal disease. The ulcers are obvious burning pain, round or oval in shape, with periodic, recurrent and self-limiting characteristics. The typical ulcer is yellow, red, concave and painful, i.e. the surface of the damage is covered with a yellow or grayish-white pseudomembrane; the periphery has about 1mm of congested red band; the center is concave and the base is soft; and the burning pain is obvious. In Chinese medicine, it is called mouth sores, mostly due to poor diet, spicy and fatty food, internal heat, which attacks the mouth and tongue through the meridians, or excessive emotions, excessive thinking, depression and fire, hyperactive heart fire, which burns the mouth and tongue. In the formula, Si Miao Wan, Tu Fu Ling clear dampness and heat, Da Qing Ye, Lei Gong Vine detoxify, Shu Da Huang pass the stool and diarrhea heat, Zhu Ye, Bai Mao Root clear the heart and diuretic, Bai Shao and Gan Cao slow down the pain, Bai He astringent sores. The combination of the herbs clears heat and dampness, and clears the bowels and detoxifies the body. 4.Gynecology Zheng, female, 28 years old, was first diagnosed in August 2009. She was 3 months postpartum and had been in general discomfort for 2 months. 2 months later, she started to experience heaviness all over her body, especially below the waist, with wandering pain in the large and small joints of the extremities, without joint swelling, and even in the morning, with fear of wind and heat, sweating when moving, and constipation and yellowish urine. His body is fat, his face is red and moist, his voice is high and strong, his tongue is red and greasy with yellow fur, and his pulse is smooth. The diagnosis is postpartum body pain with evidence of damp-heat obstruction and loss of harmony between Ying and Wei. The treatment is to clear heat and remove dampness and harmonize Ying and Wei. The medicine used is Huang Bai, Cang Zhu, Huang Qi, Dan Shen, Papaya 15g each, Job’s tear, Qing Feng Vine, Tu Fu Ling, Bai Shao 30g each, Chuan Niu Knee, Chicken Blood Vine, Lonicerae, Dou Wu 20g each, Gui Zhi, Sheng Gan Cao 6g each, 1 dose per day, after 6 doses, he reported that his whole body was as relaxed as removing heavy things, the joint pain disappeared, and the fear of wind was relieved. He was asked to continue taking one dose every other day, and then 6 doses, the symptoms were relieved. Note: Postpartum rheumatism is a common disease that occurs within 100 days after delivery (including miscarriage and miscarriage) with discomfort, pain, soreness, reappearing, numbness of limbs, muscles and joints, or mildly restricted function [1]. In clinical practice, this disease is characterized by deficiency and stasis. The patient’s evidence is complex, with fear of wind and heat, which makes it easy to judge as a mixture of cold and heat. Therefore, it is effective. 5.Urology Xiao, male, 32 years old, was first diagnosed in July 2009. He had difficulty in urination with slurry discharge from the urethra for 3 days. Examination: red and swollen urethral orifice with white plasma discharge, burning urine, urethral discomfort and itching, red tongue with thin yellow coating and smooth pulse. Laboratory examination: no gonococcus found, positive for mycoplasma. There was a history of travel before the onset of the disease. The diagnosis is gonorrhea. The treatment should be to clear heat and dampness, and to remove turbidity and toxicity. We used 15g each of Atractylodes macrocephala and Phellodendron, 30g each of Atractylodes macrocephala, Coix seed, Tu Fu Ling, Rhizoma Dioscorea, Plantain, Gong Ying and Slippery Rock, 15g each of Radix Achyranthes bidentata, Radix zedoariae, Dampi, 10g each of Radix Scutellariae and Glycyrrhiza glabra, 1 dose per day, together with the application of fluoroquinolone antibiotics. 6 doses later, there was no discharge from the urethra and urethral discomfort was relieved. After taking 6 more doses, laboratory examination: mycoplasma was negative. Press: Non-gonococcal urethritis refers to urethritis caused by pathogens other than gonorrhea, mainly Chlamydia trachomatis and ureaplasma mycoplasma, which is one of the most common sexually transmitted diseases. The treatment is to clear damp-heat with Si Miao Wan, Tu Fu Ling, Dioscorea Z, Che Qian Cao, Slippery Rock, Ze Xie, Bai Mao Root, detoxify damp-heat with Gong Ying Huang Qin and Sheng Gan Cao, cool the blood and clear heat with Dan Pi, together clearing heat and dampness, resolving turbidity and diarrhea.
