Treatment of liver cancer moves toward standardization

  Liver cancer is a serious threat to the health of the nation. China is a big country of liver disease and liver cancer. According to the 2008 “Third National Review Sample Survey Report on Causes of Death” released by the Ministry of Health of China, liver cancer ranks as the 2nd cause of death from malignant tumors in urban areas and the 1st cause of death from malignant tumors in rural areas; and the mortality rate of liver cancer is on the rise. In the age groups of 15-44 years old for men, 45-64 years old for women and 45-64 years old for men, liver cancer is the first cause of death. Obviously, liver cancer, which was known as the “King of Cancer” in the past, is still a serious disease threatening the life and health of our people.  Two factors shackle the development of liver cancer After decades of research and development, liver cancer treatment has made great progress and treatment methods are increasingly abundant, but the multidisciplinary and comprehensive treatment of liver cancer has been difficult to standardize, which restricts the overall treatment effect of liver cancer for two reasons.  There are many factors affecting the prognosis of hepatocellular carcinoma, and the clinical staging at home and abroad has not been unified. The vast majority of hepatocellular carcinoma (HCC) is developed further from hepatitis and cirrhosis, which determines that every HCC patient suffers from hepatitis, cirrhosis and liver cancer at the same time, leading to the severity and complexity of the disease. There are many factors affecting the prognosis of HCC, including tumor factors such as tumor size, number, site, envelope, degree of pathological differentiation, vascular cancer emboli, lymph node metastasis and distant metastasis; liver disease background factors of hepatitis and cirrhosis; and viral and organismal genetic factors. This makes the clinical staging of HCC has been confusing to scholars at home and abroad for many years, and it is difficult to obtain a more consistent opinion. Because the special characteristics and liver function background of HCC have not been taken into account, the TNM staging of liver cancer of the International Union Against Cancer has not been recognized, while more than ten clinical staging of liver cancer are popular internationally, mainly the TNM disease staging system for liver cancer (UICC), the Okuda staging system in Japan, the CLIP staging in Italy, the BCLC staging in Spain in Europe, the CUPI staging in Hong Kong and 2001 clinical staging of hepatocellular carcinoma by the Hepatocellular Carcinoma Professional Committee of China Anti-Cancer Association. The inconsistency of clinical staging has seriously affected the formulation and promotion of liver cancer treatment standard.  There are many treatment methods for HCC, including surgical resection, transplantation, local ablation, vascular embolization, radiation therapy, chemotherapy, targeted drugs, Chinese medicine, internal radiation therapy, bioimmunotherapy, etc. These treatment methods are divided into different disciplines and departments. According to the statistics of a large hospital in China, there are 16 departments in the hospital that can treat liver cancer patients. For example, the departments that carry out radiofrequency ablation treatment in China include surgery (general surgery or hepatobiliary surgery), radiology and imaging, interventional medicine, ultrasound and oncology. Each discipline or department carries out diagnosis and treatment according to their own standards, and most of them put their own interests first and use a single treatment method for repeated treatment, while they seldom communicate and exchange with other disciplines to seek multidisciplinary comprehensive treatment, which makes it difficult for patients with liver cancer in China to get the best method of treatment and seriously affects the treatment effect of liver cancer in China.  There is already a consensus on liver cancer treatment in China Therefore, on the basis of emphasizing comprehensive treatment of liver cancer, it has been the expectation of many scholars to formulate more authoritative clinical treatment guidelines for liver cancer. In recent years, the European Association of Liver Diseases and the American Association for the Study of Liver Diseases have unified liver cancer staging standards and published similar clinical treatment guidelines for HCC. The “Expert Consensus on Standardized Diagnosis and Treatment of Primary Liver Cancer” was formulated and published. Subsequently, under the auspices of the Medical Secretary of the Ministry of Health, two “Expert Workshops on Standardized Diagnosis and Treatment of Primary Liver Cancer” were held by domestic experts. “and proposed a multidisciplinary comprehensive treatment model for hepatocellular carcinoma suitable for China’s national conditions.  In conclusion, hepatocellular carcinoma is a kind of cancer with high malignancy, and most of them are already in the middle and late stages when diagnosed in China, and the surgical resection rate is low, and even after surgical resection, there is still a high recurrence rate. Standardized multidisciplinary and comprehensive treatment (MDT) has been proven to be an important means to improve the treatment effect of malignant tumors, and there have been breakthroughs in the treatment of several cancer tumors. However, due to the specificity and complexity of liver cancer, there is no unified and recognized clinical staging, and the treatment standard and clinical guidelines in China have been blank, which seriously restricts the improvement of clinical treatment effect of liver cancer in China and affects the survival benefit of liver cancer patients. In recent years, the publication of “Expert Consensus on Standardized Diagnosis and Treatment of Primary Liver Cancer” and the formulation of major malignant tumor standards under the auspices of the Ministry of Health will hopefully change this undesirable situation, so as to standardize the clinical diagnosis and treatment of liver cancer, reasonably carry out multidisciplinary comprehensive treatment and improve the overall effect of liver cancer treatment in China.
