Chinese medicine treatment for malignant tumors complicated by intractable diarrhea

  The effect of conventional therapy is not good; the malnutrition caused by long-term diarrhea will also further reduce the immunity of patients, which not only seriously affects the quality of life of patients, but also may lead to the smooth implementation of anti-tumor treatment measures, thus affecting the overall treatment effect [1,2]. In recent years, the author has adopted Chinese medicine therapy to treat tumor recalcitrant diarrhea and achieved better clinical efficacy, and now, without trying to be superficial, I will summarize my personal experience as follows for reference only.  1, the etiology of tumor stubborn diarrhea 1.1 tumor directly caused by the tumor directly caused by the stubborn diarrhea is mostly seen in the gastrointestinal system malignant tumor before surgery, such as intestinal cancer cancer tissue compression, stimulation of intestinal cavity, causing intestinal irritation symptoms. At the same time, cancerous tissues in these areas, if complicated with infection and necrotic ulceration, can lead to increased secretion, thus causing diarrhea.  1.2 Caused by tumor surgery After surgery of any malignant tumor, as long as the anatomical structure and physiological function of the digestive tract are changed, it can cause diarrhea. For example, too wide anastomosis after gastric cancer surgery, too fast entry of food into intestine after intestinal surgery, reduction of digestive enzymes after surgery of stomach, gallbladder and pancreas tumors, resulting in poor digestion and absorption of food, can cause diarrhea.  1.3 Caused by radiation therapy Intractable diarrhea caused by radiation therapy is common after radiation therapy of pelvic tumors, such as cervical cancer, ovarian cancer, prostate cancer, intestinal cancer, retroperitoneal tumors, etc. Radiation therapy can directly cause radiation enteritis, whose main manifestation is abdominal pain and long-term diarrhea, and conventional treatment means mostly have poor effect.  1.4 Caused by drug toxicity Modern pharmacological studies have confirmed that bitter and cold purgative and detoxifying drugs such as Scutellaria baicalensis, Huanglian, Vine’s root, Seven-leaved one flower, Half-branch lotus and Half-sided lotus have the effect of inhibiting tumor cell proliferation [2]. However, long-term administration of such drugs tends to defeat the stomach with bitter cold and injure yang and rob yin, leading to the occurrence of diarrhea. In addition, dysbiosis of intestinal flora caused by the use of antibiotics in perioperative senior patients can also cause recalcitrant diarrhea; some chemotherapeutic drugs (such as irinotecan for intestinal cancer), as well as targeted therapeutic drugs (such as Erysal and Troche) commonly used in lung cancer at present, also have adverse effects that cause recalcitrant diarrhea.  In summary, there are many causes of tumor-induced diarrhea, and the causes listed above are common clinical types. Most of the tumor diarrhea can be cured by conventional treatment. The following is only a discussion on the Chinese medicine treatment of persistent and intractable diarrhea.  2.1 The main cause of tumor stubborn diarrhea is “internal dampness”. According to Chinese medicine, the main pathological factor of diarrhea is dampness, among which there are differences between external dampness and internal dampness, and stubborn diarrhea is mostly caused by internal dampness. The main cause of stubborn diarrhea in tumor patients is internal dampness, and there are more cases with cold and yang deficiency, and fewer cases with solid and hot evidence; the lesions usually involve the spleen, liver and kidney.  2.2 Treatment is based on the core of “transporting the spleen and resolving dampness”. At this time, the treatment should be to strengthen the spleen and resolve dampness; the formula can be selected from Si Jun Zi Tang, Ginseng Ling Bai Zhu San, and Tonifying Zhong Yi Qi Tang, etc. Commonly used drugs such as Dang Shen, Prince Ginseng, Fried Atractylodes Macrocephala, White Lentil, Lotus Pulp, Ze Xie, and Coix Seed.  Spleen deficiency and dampness is the early stage of the disease, if handled properly, the condition will improve. On the contrary, if the middle earth is more deficient, the liver and wood will take advantage of it, and the evidence of liver qi stagnation or liver fire will be seen. At this time, the proper treatment should be to dredge the liver and clear the liver; the formula can be used to treat pain and diarrhea, Chai Hu Dredge Liver San, Free and Easy San, etc. Commonly used drugs such as Chai Hu, Chen Pi, Fried White Peony, Buddha’s Hand, Fragrant Herb, Gentian Herb, Scutellaria Baicalensis, Huang Lian, White Head Worm, etc.  If liver qi stagnation is not treated properly or if the disease develops further, it can be seen as spleen yang deficiency, which is evidenced by diarrhea, cold pain in the stomach and epigastrium, loss of appetite, depression, lack of warmth in the limbs, pale white tongue with teeth marks, and sunken pulse. The common method is to warm the middle of the body and disperse the cold, and the formula can be used such as Rizhong Pill and Sihui Tang. Spleen Yang deficiency, long will be and kidney. This disease can also be seen in the deficiency of kidney yang, it is appropriate to warm the kidneys to disperse the cold and consolidate the root as the main method of treatment; the formula can be selected from the Four Spirit Pill, Kidney Qi Pill, Right Return Pill, commonly used drugs such as light eucommia, cinnamon, bone marrow, Xian Ling Spleen, deer horn cream, codonopsis, etc.  If the disease develops further and reaches both spleen and kidney deficiency, the symptoms include prolonged diarrhea, failure to transform the complete grain, abdominal pain and intestinal tinnitus, preference for warmth and dislike of cold, lumbago and cold limbs, diarrhea on the fifth night, white fur, and sunken pulse [5]. The Materia Medica HuiYin contains: “Fulong liver, warming the spleen and permeating dampness, dry and calm, warm and harmonious, sweet and astringent, to hide for use”. It is suitable for stubborn diarrhea caused by various reasons, and the clinical dosage can be slightly larger (60-120g); first decocting the soup and then using it to decoct the rest of the medicine can achieve the effect of thickening the earth and consolidating the middle.  In addition, this disease can also be seen clinically with damp-heat stagnation in the intestinal tract, especially after radiotherapy. The main clinical manifestations are urgency, constipation, unpleasant bowel movements, abdominal pain, yellow tongue coating, and stringy pulse; the treatment is to clear damp-heat, regulate qi and guide stagnation, and the formula can be used as Hovenia and Scutellaria Pill, Ge Gen and Scutellaria Pill.  2.3 Emphasis on “facilitating urination” and “dispelling wind” The above treatment methods are commonly used for recalcitrant diarrhea, in addition, I also commonly use “facilitating urination” and “dispelling wind”. In addition, the author also often uses “diuretic” and “wind dispelling” to treat tumor stubborn diarrhea with satisfactory results. The main cause of stubborn diarrhea is “spleen deficiency and dampness”, and various treatment methods also start from transporting the spleen and removing dampness, among which the importance of “facilitating urination” to remove dampness cannot be ignored. “Therefore, clinical diagnosis and treatment of tumor stubbornness should be based on the treatment of dampness. Therefore, in the clinical treatment of stubborn diarrhea of tumors, we can add Che Qian Zi, Poria, Tong Cao, and Slippery Rock on the basis of evidence, which can facilitate the urine to actualize the stool, which is really effective. In addition, for the diarrhea caused by damp-heat retention, the purpose of heat can be achieved from the stool and down.  The “wind dispelling method” is a common treatment for paralysis, but how can it treat diarrhea? The wind referred to here is different from the external wind, but it is the same as the nature of “good movement and change”. Patients with stubborn tumor diarrhea often have abdominal pain, abdominal distension, intestinal tinnitus and other symptoms. The drugs used to dispel wind and overcome dampness are mostly dry in nature, and dryness can overcome dampness, which means “wind can overcome dampness” from the perspective of medicinal properties, so the “wind dispelling method” can also be used to treat diarrhea. The combination of these drugs with conventional medicines can often have unexpected effects [3].  3. Special case Xia, male, 65 years old. Diagnosis: right upper lung adenocarcinoma after postoperative chemotherapy Date of first consultation: March 4, 2010 The patient complained of “dry cough for two weeks” and was treated with anti-inflammatory and cough suppressant therapy successively with poor results. On January 20, 2010, he underwent right upper pneumonectomy and postoperative chemotherapy with Tysol plus cisplatin. He started to have diarrhea in the third cycle, which occurred tens of times a day. After repeated treatment with Flavopiridol tablets and Bifidobacterium bifidum tablets, he turned to Chinese medicine for treatment.  Diagnosis: diarrhea (dozens of times a day), thin and sometimes watery stools, no abdominal pain and post-acute heaviness; nano-accommodation, still sleeping peacefully, emaciated body, yellow face; light fat tongue, thin white fur, sunken and slow pulse. Diagnosis: Spleen deficiency and dampness; treatment: tonifying the spleen, benefiting Qi, permeating dampness and stopping diarrhea; formula with Ginseng Ling Bai Zhu San plus reduction. Take in divided doses.  After taking 14 doses, the patient’s bowel movements decreased to 2-3 times a day, and the stools became more solid. After taking 7 doses, the diarrhea was cured.  This is a sign of spleen deficiency and internal stagnation of water and dampness; the wasting of the body and yellow face are signs of spleen deficiency and lack of source of Qi and blood biochemistry, which are supported by the tongue and pulse. The formula is based on the following principles This formula incorporates a variety of treatments, but it is easy to see that “benefitting the qi and strengthening the spleen”, “rationalizing the qi and awakening the spleen”, “warming the middle and strengthening the spleen” and “tonifying the fire and helping the earth “strengthening the spleen and resolving dampness”, “distributing water-dampness” and “dispelling wind-dampness” are all based on “dampness”. “strengthening the spleen and transforming dampness”, “distributing water-dampness” and “dispelling wind-dampness” are all “dampness” oriented. We always grasp the core disease mechanism of “spleen deficiency and dampness” and use the prescriptions to achieve good curative effects.
