Proper understanding of prevention and treatment of liver cancer

  Liver cancer is divided into two types: primary and secondary, the former being malignant tumors of the liver itself and the latter being metastatic tumors. In recent years, with the development of treatment modes such as early treatment, comprehensive treatment and active treatment, the treatment effect of liver cancer has been improved to a certain extent, but in general, the prognosis of liver cancer is relatively poor, and there are still certain misconceptions in our understanding in the treatment of liver cancer.  1. Exaggerating the role of single treatment means and ignoring the significance of comprehensive treatment.  In the treatment of liver cancer, whether it is local treatment or systemic treatment, any one treatment means has its limitation. For example, surgical resection is the main means of liver cancer treatment, and the 5-year survival rate of small liver cancer reaches 60-70%, while the 5-year survival rate of large liver cancer is only 20%, while the 5-year survival rate of “resection after reduction” through hepatic artery ligation and embolization chemotherapy increases significantly. This is also true for various treatments such as embolization chemotherapy, cryosurgery and radiation therapy, which do not have 100% efficiency.  Clinical and experimental repeatedly proved: various treatment methods with proper cooperation, then “three” is better than “two”, “two” is better than a single method of treatment; various treatment methods can also be continued If the tumor is properly coordinated, “triple” is better than “duplex” and “duplex” is better than single method of treatment. The diagnosis and treatment of tumor need the cooperation of different departments.  2.We attach importance to the treatment of liver cancer and neglect the prevention of liver cancer.  Although China has long put forward the primary prevention of “waterproof, mold prevention and hepatitis prevention” and secondary prevention of early surgery and early treatment. However, it is undeniable that people’s awareness of prevention is still weak, such as the abandonment of pond water, changes in dietary habits, prevention and treatment of viral hepatitis, etc.  It is undeniable that the census system is not yet sound in China and the scope of census is still limited. There are still few cases of early diagnosis and treatment. Therefore, people still focus more on the treatment of clinical stage liver cancer.  Most primary liver cancers develop on the basis of hepatitis B, hepatitis C and cirrhosis, and in the process of cancer, patients often have abnormal liver function at the same time. At present, local treatment methods of liver cancer such as surgery and interventional therapy can directly damage liver tissues and aggravate liver function abnormalities, so if the protection of normal liver tissues is not considered at the same time of local tumor treatment, it is difficult to obtain good curative effect. In addition, if liver cancer occurs on the basis of chronic hepatitis and cirrhosis, if the treatment of hepatitis and cirrhosis is not paid attention to, even if the lesions are controlled, there is a possibility of new lesions appearing. Therefore, the treatment of liver cancer needs to consider the patient’s primary lesions and liver function in a comprehensive manner.  4.Paying attention to local tumor and neglecting quality of life Primary liver cancer has insidious onset and lacks typical symptoms in early stage, so it is difficult to be detected; most liver cancer patients are already in middle and late stage when they are found. Patients often have persistent distension or dull pain in the liver area, progressive hepatomegaly with different degrees of pressure pain, and jaundice, splenomegaly, ascites and other manifestations of cirrhosis in late stage. For patients in the middle stage, surgery, embolization chemotherapy and radiation therapy are the main treatment tools. It can control the tumor growth in some patients. For advanced stage patients, it is difficult to carry out surgical chemotherapy, radiotherapy and other treatment methods that have the effect of damaging the body’s positive energy. At this time, it is more important to pay attention to the overall conditioning of the patient, to “support the righteousness and dispel the evil”, to reduce the pain and prolong the survival time of the patient.  5.The role of Chinese medicine in the treatment of liver cancer is viewed in a discriminatory way Chinese medicine has a very important role in the treatment of liver cancer. Chinese herbal medicines that benefit qi and nourish blood can promote the recovery of the organism of patients undergoing surgery; Chinese herbal medicines that relax the liver and strengthen the spleen can reduce the toxic side effects of radiotherapy and increase the therapeutic effect of radiotherapy. The combination of Chinese herbal medicine to support the righteous and dispel the evil can reduce the symptoms of patients with advanced disease and increase their chances of long-term survival with tumors. However, we should not exaggerate the therapeutic effects of TCM, and we should not exaggerate the efficacy of individual treatment methods. The blind and massive application of “attacking poison with poison” and “activating blood circulation and removing blood stasis” in advanced liver cancer patients may lead to aggravation or even deterioration of the disease. For example, excessive application of “activating blood circulation and removing blood stasis” may lead to hemorrhage and induce liver coma. Therefore, only through the combination of disease identification and evidence identification, the combination of local and overall, the combination of Chinese medicine and Western medicine, can we achieve good results.  6.Neglect the role of psychotherapy.  Tumor patients’ body and mind are greatly challenged. They need the understanding and help from the society, family and medical personnel. However, people often neglect the spiritual encouragement and psychological help to patients while actively treating cancer. It is an objective fact that liver cancer develops rapidly and has poor prognosis, however, there are many clinical cases that survive for a long time after comprehensive treatment, therefore, doctors should encourage patients to actively cooperate with treatment and strive for the best results.
