This past March 30 was World Bipolar Disorder Day. Bipolar disorder is a strange disease in which emotions swing like a “pendulum”, sometimes the patient is full of energy, the sky is free; sometimes depressed, unable to do anything. When someone has repeated “mood swings” and “roller coaster” like mental state, you need to be alert to a “pendulum” disease – bipolar disorder. -Bipolar disorder. Bipolar disorder is also known as bipolar disorder, the symptoms are both manic or hypomanic episodes, but also depressive episodes of a class of mental disorders. According to the World Health Organization, the global prevalence of bipolar disorder is about 1%-2%, up to 5%. But in recent years, he seems to feel that his mood and body have changed a lot, like a “warm man” when he is mild, and like a “demon” when he is irritable. Even the mood changes greatly during the day, often at night when the creative thinking is quick and refreshed, but in the daytime becomes unresponsive, depressed, and once thought of suicide lingered …… This strange mood like a “pendulum” wavering The disease, psychologically known as “bipolar disorder”, was also called manic-depressive disorder. Patients alternate between manic and depressive symptoms, and sometimes they are mixed at the same time. And these people’s suicidal thoughts are often very hidden, family and friends are difficult to find. The latest data show that the lifetime risk of “pendulum” disease in China is higher than 1%, and more than 2/3 of patients have symptoms before the age of 20, but it has not attracted enough attention. In order to raise public awareness of bipolar disorder and to improve the level of treatment for bipolar disorder in China, World Bipolar Disorder Day has officially come to China under the advocacy of experts in the field of psychiatry nationwide. Bipolar disorder occurs in adolescents, and its death and disability rates are even as high as those of depression. According to a survey, the point-in-time prevalence of bipolar disorder is 1-4%, and the lifetime prevalence is even higher. The peak age of onset of the disorder is mostly between 15-19 years old, 25-50% of patients have experienced suicidal behavior, and nearly 20% of patients die by suicide. Bipolar disorder has the highest rate of suicide among psychiatric disorders and is likely to cause lifelong regret if not detected early and treated promptly. The average time from first onset to diagnosis for patients with bipolar disorder is 10 years, and is characterized by high relapse rates, high disability rates, high mortality rates and low diagnosis and treatment rates. “Fifty percent of patients in China are not treated within five years of onset and 36 percent are not treated within 10 years of onset.” Patients with bipolar disorder are easily misdiagnosed It is important to note that although patients with bipolar disorder often act depressed, if they are treated as depression, it will only make them more unstable and even worse. Moreover, patients should not stop the medication immediately when they see the symptoms improve, because if they do not follow the medical prescription for standardized treatment, the relapse rate of the disease within one year is as high as 70%. Distinguish whether it is a mild manic episode or a personality that makes the difference, can be distinguished from these aspects: bipolar disorder patients with mild manic episodes can exist for several days of high mood, high energy, a significant sense of good self, inattention, mild profligacy, increased social activities, and increased sexual desire. Sometimes this is manifested by being easily irritated, dropping things, being conceited and arrogant, and behaving more recklessly. It is because many patients exhibit mild mania and do not overly concern themselves or their family members, resulting in patients with bipolar disorder being easily misdiagnosed as depression when they present as a depressed state for consultation. According to statistics, about one-third of bipolar disorder patients nationwide are treated as depressed patients at the initial diagnosis. From the perspective of the affected population, the average age of onset of bipolar disorder patients is 30 years old, while the average age of monophasic depression is 40 years old, and, the first onset of depression before the age of 25 is more likely to be bipolar disorder. Generally depressive symptoms last more than 14 days, and low mood will not change due to external environmental stimuli, and intermittent manic or hypomanic symptoms for more than 4 days, we should consider seeking help from the relevant professional doctors. Treatment needs to focus on the “three principles” of bipolar disorder is mainly caused by biological reasons, medication is the basis, the need to choose a stable state of mind medication to break the emotional wavering circle. Some patients receive unreasonable treatment of “headache treatment, foot treatment”, anti-manic treatment when manic episodes, and antidepressant treatment when depressive episodes, resulting in further deterioration of the condition. If bipolar disorder is suspected, patients need to go to mental health professional institutions for clear diagnosis and standardized treatment as soon as possible, so as to control the disease in time, reduce relapse and avoid deterioration. The treatment of bipolar disorder needs to focus on “three principles”: the principle of integration, the principle of long-term, and the principle of joint participation of patients and families. The principle of integration refers to the use of multiple tools, such as psychotropic medications, physical therapy, psychotherapy and crisis interventions, to improve the efficacy and compliance; the long-term principle refers to the need for patients to follow medical advice for long-term treatment to achieve coexistence with the disease and improve the quality of life; and the principle of patient-family involvement, which encourages family members to participate in the treatment process to help patients improve compliance and reduce relapse. Bipolar disorder is a disease that lasts a lifetime. Pharmacological treatment (including mood stabilizers, antipsychotics, and antidepressants) is still the main treatment for bipolar disorder, and mood stabilizers (such as valproic acid and lithium carbonate) are the main internationally recognized treatment drugs, the main choice from the acute phase to the consolidation and maintenance phase, and the first-line drugs for the long-range treatment of bipolar disorder with their long-lasting effects and low side effects.
