Headache is a common condition encountered in clinical pain management and has many causes. One type of headache with neck pressure pain, often related to cervical nerve irritation, has a high incidence, a complex clinical presentation, a long duration of headache, and is more difficult to treat, and is now called cervicogenic headache. The main causes of cervicogenic headache include the following two aspects. 1. The posterior branches of the 1st-4th cervical nerves in the cervical spine are connected to each other to form a ring, and together they form the greater occipital nerve, the lesser occipital nerve and the greater auricular nerve, which innervate the top of the head, the back of the occiput, the auriculotemporal region and other related areas respectively. Under the circumstances of long-term head lowering, chronic strain and poor posture, the posterior branches of the 1st-4th cervical nerves of the cervical spine are strained and compressed, resulting in pain in the top of the head, posterior occipital and auriculotemporal regions, and extending to the whole head, forming a typical cervicogenic headache. For intractable cervicogenic headache, in addition to drugs, nerve block can often achieve satisfactory treatment effect, including cervical 2 transverse process block, cervical 2-3 spinal nerve posterior branch block, cervical 2 dorsal root ganglion block, etc. Generally, 3-4 times of block can achieve satisfactory effect. 2.Cervical degeneration In the case of cervical degeneration such as cervical spine hyperplasia, instability, straightening of curvature and retroflexion, the cervical vertebral artery and carotid artery will be strained or compressed, causing spasm of the vertebral artery and carotid artery, resulting in headache caused by cerebral ischemia. For this type of headache, in addition to medications, stellate ganglion blocks often achieve satisfactory results. Stellate ganglion block can relax the sympathetic nerve and in turn dilate the vertebral artery and carotid artery to improve the blood supply to the brain, thus significantly relieving the headache. The role of nerve block in cervicogenic headache is now gaining attention as an important treatment for intractable headache in China and abroad.
