Focusing on standardized treatment of hepatitis C Improving cure rates

Recently, we have admitted some patients with chronic hepatitis C who relapsed soon after the completion of antiviral treatment, and analyzed their previous treatment and found that most of the patients’ relapses were due to non-standardized treatment. In fact, if standardized treatment, the vast majority of patients with hepatitis C can be cured, and the standard of cure is that HCV RNA can always remain negative six months after stopping the drug. Hepatitis C usually progresses slowly, the symptoms are not obvious, so it is often used “quietly” to describe the characteristics of its onset, but after infection with hepatitis C virus, about 50% -80% of patients will develop into chronic hepatitis or even cirrhosis and liver cancer, the incidence of cirrhosis after 20 years of infection is about 10% -15%. The incidence of cirrhosis after 20 years of infection is about 10%-15%. Although the prognosis for Hepatitis C is poor, thankfully it is now a curable disease and treatment options are the same worldwide. Hepatitis C virus has different genotypes, and genotype I usually has a poorer prognosis and poorer response to treatment, while most of the viral genotypes of Chinese chronic hepatitis C patients are genotype I. Therefore, it was previously believed that the treatment of hepatitis C in China was ineffective. However, in recent years, more and more clinical data show that the cure rate of hepatitis C in Chinese people is higher, why? It turns out that the therapeutic effect of hepatitis C is not only related to the genotype of the virus and other factors, but also related to certain chromosomal genotypes of the infected person, and the genotype of the virus is more important than the genotype of the infected person. At present, it is very clear that if the IL-28B genotype on chromosome 19 of an infected person is CC, the durable response rate after treatment is usually high, that is to say, the cure rate is higher, and more than 70% of the Chinese people are CC, so the cure rate of Chinese patients with chronic hepatitis C will be higher. However, I often see some patients with hepatitis C who relapse soon after stopping medication. By analyzing the reasons for relapse, I found that most of them are caused by irregularities in the treatment, including the type of medication, the dosage of medication, the method of using medication, and the course of treatment. As the treatment of hepatitis C is long and costly, and once relapse, it is necessary to restart the treatment, which on the one hand increases the economic burden, and at the same time, the disease will also progress, so it is recommended that patients should be under the guidance of experienced doctors to carry out regular treatment.
