Second surgery for recurrent glioma

  Recently, we have received several consecutive patients with glioma, all of them after surgery in local hospitals. They inquired whether they could have another surgery. It is difficult for me to answer.  Glioma is a malignant tumor that occurs in the brain. According to the guidelines of the Chinese Medical Association, the first surgery is to remove as much of the tumor as possible with safety, and the appropriate radiotherapy and chemotherapy regimen is chosen after the surgery according to the pathology report. Even if the treatment is very successful, the tumor will still recur in a certain period of time. After recurrence, to open or not to open?  My personal understanding is to consider from the following aspects: 1. the condition of the tumor itself: what grade is the pathology? If it is grade 4 glioblastoma, the tumor recurrence is in the deep part, it is not recommended to operate again; grade 2-3, superficial recurrence is recommended to operate again.  2. The patient’s own condition: advanced age, poor general health condition, and no request from the patient, reoperation is not recommended.  3, the situation of the family: whether they fully understand the condition, have a clear understanding of the whole disease process, and come up with their own opinions in combination with the socio-economic status of their own family.  In conclusion, reoperation for recurrent glioma is an individualized choice, so be careful and cautious again!
