Treatment of giant neurofibroma in the lumbosacral region

  On December 7, 2010, a special patient was admitted to the Plastic and Reconstructive Surgery Clinic, named Zhang Shaocheng, male, 20 years old, a college student of a medical university in Anhui Province, with a 160cm height but a large “buttock”. It was only after the examination that the large “buttocks” was actually a huge neurofibroma.  This tumor has been with Zhang Shaocheng for 10 years, the young man is not tall, the body l small, but this tumor is really “vigorous” growth, during this period, Zhang Shaocheng infinite trouble, the tumor grows bigger and bigger, like a huge bag on the back and buttocks, seriously affect his life and study, and a heavy psychological burden, since he was 10 years old. Since he was 10 years old, his parents took him around to seek medical treatment and went to many famous hospitals across the country, but every time he went with hope and returned with disappointment. This time he came to the PLA General Hospital with the last hope and decided to give it a try.  In the outpatient clinic, Dr. Xu Jun, the chief physician, conducted a comprehensive and detailed examination of Zhang Shaocheng, explained his condition to him and his parents carefully, and finally admitted to the plastic surgery department. After admission, Dr. Han Yan, together with Dr. Xu Jun, organized a case discussion with all the doctors in the department to determine the surgical plan to remove the mass. However, there was another problem before the surgery, which was blood transfusion. The bleeding during neurofibroma resection was very dangerous, and without sufficient blood supply, the surgery could not be performed. After asking the director of the transfusion department for a consultation, he said that he would actively cooperate with us in the surgery and prepare an adequate blood supply for Zhang Shaocheng.  With sufficient preoperative preparation, the surgery started at 10:00 on December 14, 2010. The surgery was very difficult due to the huge tumor, vicious intraoperative bleeding, but after 10 hours of struggle by Director Xu Jun, Director Han Yan, Attending Physician Guo Lingli, Attending Physician Yang Hongyan, Dr. Liu Dai, and Dr. Ren Jing, and with the great cooperation of the treatment team of the Anesthesia Department led by Director Liu and Chief Nurse Xu of the operating room, the treatment team of the Anesthesia Hand Center Finally, the huge neurofibroma behind Zhang Shaocheng, weighing 14.5 kg, was largely removed.  After the operation, Zhang Shaocheng’s vital signs were not stable and his hemoglobin was once as low as 5g/ml. In order to protect Zhang Shaocheng’s life and speed up his post-operative recovery, Zhang Shaocheng was transferred to the surgical intensive care unit for treatment. That night, Director Song Qing of the Surgical Intensive Care Unit formulated a rescue plan and led his comrades in the department to actively carry out rescue until the early morning of the next day, so that the patient’s condition turned to safety. Every day after the operation, Director Han Yan and Director Xu Jun led the doctors in the department to check the room in the intensive care unit, because the surgical wound exuded a lot of fluid, it was necessary to change the medication every day, but the patient was weak after the operation and could not cooperate, so it took 4 or 5 physicians to help turn over to complete the medication change, each time the medication change was sweating, but seeing Zhang Shaocheng’s stable condition, we all had no complaints. 8 days later, in the surgical intensive care unit With the joint efforts of all physicians, Zhang Shaocheng successfully passed the dangerous period, the vital indicators gradually stabilized, December 22, 2010, Zhang Shaocheng transferred back to the plastic repair department to continue treatment.  Just returned to the department of the first day, Zhang Shaocheng because of the previous had not defecate, after giving open plug anal, Zhang Shaocheng that defecate to bed. However, the orthopedic nurses and doctors did not complain and changed the bed sheets and dressings for him in time. Under the careful care of the plastic and reconstructive surgery staff, Zhang Shaocheng was able to move freely and was happily discharged from the hospital on January 21, 2011 after treatment such as drug changes, local implants and anti-infection.  After 45 days, he was successfully treated for a huge lumbosacral neurofibroma in collaboration with the Department of Plastic and Reconstructive Surgery, the Intensive Care Unit, the Department of Anesthesiology and the Department of Blood Transfusion of 301 Hospital. Zhang Shaocheng also finally got rid of this “big burden” that had been dragging him down for 10 years.
