The combination of acupuncture and medicine has been used since ancient times, it is said: one needle, two moxibustion, three medicine, is a doctor should be the skill. The Yellow Emperor’s Classic of Internal Medicine says: micro-needle treatment of its external, soup and liquid treatment of its internal”. Acupuncture and moxibustion belong to the external treatment method, good at unblocking the meridians and collaterals, regulating qi and blood; medicine belongs to the internal treatment method, long coordination of the internal organs, support the righteousness and eliminate evil. In the internal and external gynecology and pediatrics, according to the disease, the reasonable application of acupuncture techniques, combined with the flavor of drugs, can achieve a complementary comprehensive treatment effect, and effectively improve the effectiveness of the treatment of disease. In recent years, I have used the combination of acupuncture and medicine to treat many cases of vascular headache, and achieved good results. I briefly report them as follows. Since the head is the meeting of all the yang, the house of the essence, and the place where the marrow sea resides, it is connected with the internal organs by meridians and the external world by all the orifices. As stated in the “Classes of Treatment”, “The head is the image of the sky, and all the Yang scriptures will be gathered, if the six gases are invaded externally, the essence is paralyzed internally, depressed in the empty orifices, and the pain is made when the clear yang is not transported.” Therefore, both external and internal injuries may cause headache. If the headache is caused by wind-cold, the headache is sometimes made, when cold is encountered, the pain is even in the neck and back, accompanied by nasal congestion and floating pulse, the treatment is to disperse the wind-cold. If the headache is caused by wind-dampness, the body is heavy and tired, the pulse is moist and slow, and the tongue is white and greasy, the treatment is to dispel wind and disperse dampness to relieve pain, and the formula is Qiangwu Shengshui Tang with addition and reduction, and the acupoints are mostly chosen from Yinlingquan and Sanyinjiao. If the headache is caused by internal injury, if the headache is due to hyperactivity of liver and yang, bitterness in the mouth and tinnitus, irritability of the heart, or a history of hypertension, the formula uses Tianma Hooked Vine Drink to pacify the liver and subdue yang, and the acupuncture points are mostly Tai Chong, Man Xi, Xingma and Tai Xi. In the case of blood stasis headache, the headache has been fixed for a long time, the site is stubborn and difficult to be cured, the tongue is dark red or has petechiae, the veins under the tongue are stagnant and the pulse is astringent. In addition, the course of vascular headache is generally long. In recent years, it has been found that its onset is related to abnormal clotting of human platelets and abnormal human blood viscosity ratio. According to Chinese medicine, this disease, whether due to external sensation or internal injury, involves disturbance of the clear air, loss of harmony in the veins and channels of the brain, rebellion of the meridians, stagnation of the qi and blood, and pain when it does not pass, so the dose of Chuanxiong can be increased to 30 grams to achieve the effect of activating the blood and opening the channels. As the head is the meeting place of all the meridians, as “Lenglu Medical Talk” says: “The headache belongs to the sun, from the back of the head up to the top, its pain even Xiang; belongs to the Yangming, up to the eye beads, pain in front of the forehead; belongs to the Shaoyang, up to the two horns, pain in the corner of the head. The Sun meridian travels behind the body, the Yangming meridian travels before the body, and the Shaoyang meridian travels to the side of the body. The pulse of syncope, will be in the top, so pain in the top. Although Taiyin and ShaoYin meridians do not go up to the head, but phlegm and qi are in the diaphragm, so the qi in the head cannot be released and it also hurts.” From this we know that in addition to the treatment of headache according to the identification of Chinese medicine, we can also treat it according to the pain area, following the meridians. For example, if the pain is in the forehead, it is blamed on Yangming, the medicine is Bai Zhi, and the acupuncture points are Hegu and Neiting; if the pain is on the temporal side, it is blamed on Shaoyang, the medicine is Chuanxiong, and the acupuncture points are Waiguan and Foot Linwu; if the pain is on the top, it is blamed on Jueyin, the medicine is Wu Yu, and the acupuncture points are Taichong; if the pain is in the back of the head, it is blamed on Sun, the medicine is Ligusticum, and the acupuncture points are Kunlun. On the basis of his in-depth study of the Nei Jing, Wu Kun systematically expounded the view that “the two ways of acupuncture and medicine are not mutually exclusive” in his Six Sets of Punctuation for Acupuncture. In the clinical evidence, according to the specific circumstances of the disease, combined with the strengths and weaknesses of the needle and medicine, when the needle, then the needle, when the medicine, then the medicine, when the needle and medicine together, then the needle and medicine at the same time. Needle and medicine together, in order to achieve better therapeutic results.
