Gout is a “disease of affluence”, the incidence of which is increasing day by day in China. This is because people’s standard of living is getting higher and higher, the daily intake of purine is also increasing (mainly from meat, seafood, mushrooms, beer is also high), and these purines will be metabolized in the body to produce uric acid; uric acid produced by metabolic activities in the human body, accounting for about 80% of the total amount of uric acid in the body every day. When the rate of removal of this uric acid is lower than the rate of its synthesis by intake, the uric acid content in the body will increase, and a blood test will reveal that the blood uric acid value exceeds the normal value (7mg/dL, 420μmol/L), which means that “hyperuricemia” occurs. These uric acids form urates in the blood, and at high concentrations, they may be deposited in various parts of the body with the blood, forming uric acid crystals, and the immune cells in the body see these crystals as “enemies” and gather around them in an immune response, resulting in local inflammation and severe pain, which is called gout. –This reaction occurs in about 10-15% of patients with hyperuricemia. Because blood circulation slows during sleep, urate deposits tend to occur, and the most common site of deposits is the joints of the fingers and toes (especially the big toe), so gout attacks tend to occur at night, with the big toe being the most common site of pain, and many patients often come to the doctor with severe toe pain. Although joint pain is the most common manifestation of gout, if there are only one or two attacks, it will only cause some short-term pain; if not actively treated to prevent recurrent gout attacks, this inflammatory response will destroy the joint structure and the joint will gradually deform to the point of losing function. The kidneys are the second victim, because most of the uric acid needs to be excreted through the kidneys, too much uric acid salt will be deposited in the kidneys to form uric acid crystals, directly damaging the kidneys, increasing the burden on the kidneys, making the urine creatinine rise, and gradually decreasing kidney function, thus creating a vicious circle. In addition to joints and kidneys, urate crystals are often deposited in the auricles and other soft tissues of the skin, where the skin is raised and prone to breakage and secondary infections. Therefore, the danger of gout is not simply pain for a few days, it can cause damage to our body organs, so in order to protect the joints, kidneys and other organs, it must be actively treated. Because gout must be preceded by a process of hyperuricemia, the control of blood uric acid is very important.
