Life-threatening paraquat poisoning

Most of the patients actually did not really want to die, but drank paraquat just for a moment, or to scare their friends or family. So how toxic is paraquat? A. Paraquat poisoning know more drops! Paraquat is a highly effective non-selective contact herbicide, which is very toxic to humans and animals, and can cause acute poisoning by accident or self-inflicted. The lethal dose for adults is 5~15ml. Paraquat is absorbed through the digestive tract, skin and respiratory tract, and its toxicity affects many organs of the body, which can lead to multi-organ insufficiency syndrome in serious cases. The lungs are the main target organs, and paraquat will cause the most damage to the lungs, causing pulmonary fibrosis and eventually the lungs will not be able to breathe at all – suffocating, which is also the main cause of death from paraquat poisoning, with a death rate of 50% to 70%. Paraquat was originally colorless and tasteless, but to prevent accidental ingestion, it was made into a very disgusting dark green color, and with the addition of emetic ingredients. Paraquat is scary because it has no antidote. Secondly, when you first drink it, you will feel nauseous and have no other special feeling, but slowly, you will have difficulty in breathing, and it will get progressively worse. There is no special antidote for acute paraquat poisoning. Early measures to remove the poison entering the body are the basis for successful treatment of acute paraquat poisoning. The main measures are emetic, gastric lavage and adsorption, diaphoresis, cleaning, etc. (1) Emetic and gastric lavage Stimulate the throat to induce vomiting, and lavage the stomach in a short time. (2)Inducing diarrhea Use 20% mannitol or magnesium sulfate to induce diarrhea to promote the discharge of intestinal toxins and reduce absorption. (3) Washing For skin contacts, immediately take off clothes contaminated by paraquat or vomit, wash skin and hair thoroughly with water and soap and water, don’t cause skin damage to prevent increase of poison absorption. Paraquat eye contacts need to rinse with flowing water for 15 to 20 minutes. 2.Promote poison discharge (1)Rehydration: Proper rehydration combined with intravenous diuretics. (2) Blood purification : Hemoperfusion and hemodialysis are the common methods to remove the poison from blood circulation. Hemoperfusion should be carried out as soon as possible after oral paraquat poisoning, and the effect is good within 2 to 4 hours. Hemodialysis is mainly used for patients with paraquat poisoning combined with renal impairment, and in serious cases, CRRT (24-hour continuous hemodialysis) is also performed, which costs an average of 5-10 thousand yuan per day, and even if it can be saved in the end, it costs hundreds of thousands of yuan, but most of them are empty of human materials. 3. Drug therapy Clinical application of drugs is mainly to prevent and control target organ lung damage, commonly used drugs including glucocorticoids, immunosuppressants, antioxidants, etc. 4. Support symptomatic treatment (1) Oxygen therapy and mechanical ventilation: Acute paraquat poisoning should be avoided by routine oxygen administration. (3) Nutritional support: For acute paraquat poisoning who are fasting due to serious gastrointestinal damage, give parenteral nutritional support and deep venous high nutrition. (4) Symptomatic treatment. C. Prevention of paraquat poisoning Strengthen the propaganda to make people understand the fatal toxicity of paraquat: there is no effective drug, high lethality, a small amount (5ml) taken orally can cause death, pulmonary fibrosis is the main cause of its final death and other knowledge to make people fear paraquat and stay away from it. Strengthen the monitoring of paraquat products and reduce the concentration. Ensure that the addition of malodorant and emetogenic agent is qualified to reduce the absorption after accidental administration and reduce the degree of harm. The unused paraquat solution should be recycled in time. Paraquat solution at home should be kept in a safe place, not just put it in a place where you can easily see and get it, especially not mixed with common drugs or food, to avoid children and young children from taking it by mistake and contacting with high-risk people. Paraquat is so toxic that our country has withdrawn the registration and production license of paraquat aqueous from July 1, 2014, and stopped production. Paraquat can kill a hundred grasses; if you get it, you can’t protect your life! Cherish your life, stay away from paraquat!
