Gout is a group of diseases characterized by long-term disorders of purine metabolism and increased blood uric acid leading to tissue damage. It is characterized by hyperuricemia, recurrent acute arthritis, gout stone formation, chronic arthritis and joint deformity, as well as the development of renal uric acid stones and gouty renal parenchymal lesions later in the course of the disease. The disease can occur in all age groups, but middle-aged men are the majority, 40-50 years old is the high incidence of gout, and there are reports of a younger trend of gout. The clinical manifestations can be divided into: 1, asymptomatic phase: only remain in the state of hyperuricemia. 2, acute attack phase: sudden onset, most patients suddenly feel severe joint pain, accompanied by fever, the most affected joints bunion, especially the first metatarsophalangeal joint, accounting for about 50%, followed by ankle, wrist, knee, elbow joints, etc., the shoulder and hip joints are rarely affected. The most typical symptom is the pain in the red and swollen area, which is like pins and needles, like a cone poke, and seems to feel like the bones are displaced and cannot be touched or moved. Generally, the pain gradually subsides after a few days or weeks, and joint movement can be fully restored, which often makes people think that the disease has been cured and lose awareness. However, in the following months or years, there will be again, many times alternating attacks, causing lifelong distress. 3. Intermittent seizure period: After several seizures, the interval between seizures will be shortened and the duration of seizures will be lengthened. The joints are repeatedly stimulated and finally deformed, and if the gout stones are accumulated, chronic gout will be formed. When the disease progresses, uric acid deposits in the joints gradually increase, the attacks become more frequent, and the number of joints affected by multiple attacks also gradually increases, and the swelling and pain after the attacks often do not disappear completely, and at this time has entered the chronic phase. Gout stones are usually found in the joints and kidneys. Gout stone nodules in the joints break down and form fistulas that do not heal easily, and white uric acid crystals are discharged. In addition to clinical symptoms, the diagnosis of gout can also be based on physical and chemical indicators: uric acid in the blood is greater than 7 mg/dl, and in women, it is greater than 6 mg/dl, which is considered high blood uric acid; the gold standard is to observe uric acid crystals in the fluid in the joint cavity under a polarized light microscope, and to observe joint X-rays, which can show the corresponding joint damage. In clinical treatment, we can also see patients with gout attacks but normal blood uric acid concentration, which should be taken seriously and treated according to the evidence. The principle of Western medicine is to reduce the production and increase the excretion of uric acid. The main drugs include colchicine, allopurinol, gout lisian, uric acid excretors, and anti-inflammatory and analgesic agents. In addition, strict dietary control, drinking more water and adopting a scientific lifestyle are necessary.
