When we talk about affluenza, people will think of high blood lipids, fatty liver, etc. However, in recent years, gout is chasing some affluent families and affecting the lives of middle-aged and elderly people. In the past two years, with the improvement of people’s living standard and the change of diet structure, the incidence of gout in China has also increased accordingly. Many people only know about gout after they have developed symptoms, so what kind of disease is gout, how is it caused and how can it be prevented? What is gout Gout is a crystal-related arthropathy caused by monosodium urate (MSU) deposition, a group of genetically related disorders of purine metabolism. It is clinically characterized by hyperuricemia and the resulting gouty arthritis, gouty stones and kidney lesions. Gout attacks often begin after a drinking party, with sudden onset of severe pain, redness, swelling and fever in the toe joints in the middle of the night. The first attack affects the bunion in 60% of cases, but can also involve other joints and recur. In patients with chronic gout, acute attacks can be triggered by mental stress, overexertion, joint injury, trauma and infection, and improper diet. The incidence of gout is higher in middle-aged obese male brain workers and increases with age, and in women it occurs after menopause. The most important biochemical basis of gout is hyperuricemia. Normal adults produce about 750mg of uric acid daily, 80% of which is endogenous and 20% exogenous. These uric acids are metabolized by the intestines and excreted by the kidneys to maintain the stability of uric acid levels in the body, and any problem in any of them can lead to hyperuricemia. The common occurrence of gout due to diet is a typical case of excessive production of exogenous uric acid beyond the metabolism of the kidneys and intestines. The development of gout is related to gender, diet and genetics, but a high purine diet is only one of the factors. When excessive uric acid production or reduced uric acid excretion due to kidney disease exceeds the body’s ability to remove it, it causes an increase in blood uric acid, which leads to the formation of urate crystals and deposits in body fluids and other tissues, resulting in an inflammatory reaction. Therefore, patients with kidney disease should pay attention to their diet, poor excretory capacity of the kidneys with high purine diet, it is easy to exceed the body’s ability to compensate, the formation of hyperuricemia. The purine content of animal offal, bone marrow, fish, shrimp, crab, chicken and duck is high, and alcohol can also induce hyperuricemia. Primary and Secondary From the above pathways, gout is divided into primary and secondary. In primary cases, most patients are genetically predisposed and have a primary increase in uric acid production or a decrease in the kidney’s ability to excrete uric acid, resulting in the retention of uric acid in the body. In secondary cases, most of the patients have a dysfunctional organism caused by other diseases, which leads to high uric acid. The typical first attack of gouty arthritis is usually monoarthritic, with the first metatarsophalangeal and bunion joints being the most common. In the acute phase, the onset of gout is sudden, often at night, and the patient may wake up with pain and be unable to sleep through the night. The symptoms usually resolve in 1-2 weeks. Local skin redness turns to brownish red and gradually returns to normal. Flaking and itching may sometimes occur. In the chronic phase, sodium urate deposition in the joints gradually increases, episodes become more frequent, intervals shorten, more joints are involved, pain increases, inflammation does not completely subside, gout stones appear, gout stones are more common in the joints and kidneys, gout stones can also appear in the outer ear, metatarsal toes, interphalangeal and metacarpophalangeal joints, etc. With the increasing deposition of gout stones, resulting in joint hypertrophy, deformity, stiffness and restricted movement is not The gout is a very important part of the body’s life. Gout patients should choose low purine food, eat less seafood, beef, alcohol, beans (tofu, etc.), mushroom food, do not drink beer, can drink a small amount of red wine. 2, control the intake of fat and protein to prevent the occurrence of gout and aggravation of symptoms. Gout is closely related to obesity and hyperlipidemia. Therefore, control the intake of fat and protein to prevent the occurrence and aggravation of gout has a certain role. The patient’s diet should be based on light alkaline vegetarian food, can eat more milk and eggs containing low purine, gout patients’ protein intake should be limited to less than 0.8-1.0 grams per kilogram of body weight per day, fat intake should be controlled to less than 60 grams per day, prohibit eating animal oil. 3, drink more water is conducive to the discharge of uric acid. Gout patients should drink more water, which is conducive to the excretion of uric acid, the daily water intake should be more than 2 liters. Wine (especially beer), strong tea and coffee should be avoided. For patients with frequent attacks, uric acid levels should be strictly controlled at normal values or even lower. It is recommended to adhere to medication like hypertension and keep uric acid levels under control all the time, followed up by a specialist, not to be seen once here and once there.
