The treatment of intrahepatic bile duct stones is mainly surgical. The results of various treatment methods are not fully satisfactory, and the key problems are high rate of residual stones, high rate of reoperation, and liver function damage leading to liver failure. Different methods are chosen according to different conditions: 1) Peripheral intrahepatic bile duct stones with no obvious symptoms do not require surgical treatment. (2) If the left lobe of the liver has bile duct stones without extrahepatic dilatation or stones, the left lobe of the liver should be resected. (6) For whole liver stones, if the stones cannot be removed and the liver function is life-threatening, liver transplantation can be performed. In case of combined cholangitis, antibiotic treatment should be used to control the infection. In case of severe infection, the bile duct should also be explored surgically to release the obstruction, remove the stone and drainage, and the lesion can be removed at the same time if conditions permit.
