Diagnosis and treatment of hemangioma

Hemangioma is a common disease in infants and children, with an incidence of about 1.2 to 2.6%, of which about 40% are detected within the first month of life. What is commonly referred to as hemangioma is a generic term for the abnormal growth or malformed development of all blood vessels, occurring mainly on the surface of the body, but also in deeper tissues and organs. More than half of them are head and facial hemangiomas, and more than 90% of them are infants and children. Many parents come to the doctor and say, “My child has strawberry hemangioma, doctor, please take a look at ……”, which shows that most parents have already had some understanding through the Internet and tools before visiting the doctor. Wine spots, strawberry hemangioma, cavernous hemangioma, and trapezius hemangioma are all traditional morphological classifications. At present, we classify hemangiomas into two major categories. Most of them (80%) are of the tumor type, and the tumor type is caused by a kind of vascular proliferation. The other type is vascular malformation, where the number of blood vessels does not increase, but the vessels are deformed and widened. As a result, parents inevitably blame themselves and are curious about what causes hemangioma. As parents, we doctors understand that there are many factors that influence the occurrence of hemangioma, and there is no recognized and definite statement. It is generally believed that parental heredity, parental endocrine disorders, diseases such as hypertension, environmental pollution or microbial infections during pregnancy can affect fetal vascular development. The diagnosis of body surface hemangioma is not difficult The key is to distinguish between true hemangioma and vascular malformation. These two different types of hemangiomas should be treated accordingly, and deep tissue and organ hemangiomas should be excluded, such as scalp hemangiomas, for which MRI is recommended to clarify the presence of intracranial hemangiomas. Treatment Hemangioma can be treated by observation, surgery, oral medicine, radiotherapy, freezing, laser, electrocautery, sclerotherapy injection, copper needle and intervention according to the situation. With the advancement and development of medical treatment, there are non-surgical treatment options for hemangioma, and most hemangiomas can be cured perfectly with non-surgical options. Each treatment method has its own advantages and disadvantages, which should be considered based on factors such as whether it is a true hemangioma or a vascular malformation, the patient’s age, the size and location of the hemangioma, the growth rate, and prior treatment. Observation is suitable for hemangiomas in non-functional areas with small scope and no obvious growth. Laser treatment is applicable to the treatment of sensitive hemangioma. At present, laser technology is advancing rapidly, and the treatment range is getting wider and wider, and the technology is getting more and more mature, and the types of hemangioma suitable for treatment by various lasers are also different. However, laser is not a panacea, and laser treatment may leave serious pigmentation and scarring, so caution should be exercised for facial hemangiomas. Sclerotherapy injections have been gradually accepted by patients and families in recent years. The most used sclerosing agents are pinyamycin and anhydrous alcohol, and polyglaucine is a new sclerosing agent with fewer side effects. Sclerosing agent can be treated alone or with other treatment modalities. It is flexible in use and has few side effects, but requires multiple treatments, while there are problems such as tissue mechanization in the injection area. Oral medication for hemangioma is a non-invasive treatment. In the past, oral prednisone tablets were used for treatment, and in recent years, oral insulin treatment for hemangioma has been carried out. Many parents have also learned about it to some extent through the Internet and professional books, and they are half-hearted: one is a hormone drug and the other is a drug for heart disease, is it effective for hemangioma treatment? Our answer is yes, and this treatment is the most favored by domestic and foreign experts. This is not the first time that hormones are used to treat hemangioma. Dexamethasone was first used as a local injection to treat hemangioma and achieved good results. Doctors can responsibly tell every parent that it is safe and that the characteristic signs caused by taking hormones will gradually disappear after stopping the medication. As for freezing, radiotherapy and electrocautery, these therapies are rarely used in clinical practice due to their lack of efficacy and side effects. Surgery is still the most effective way to treat hemangioma, but it is not the first choice. Surgical resection is only used when other non-surgical treatments are ineffective or when the surgery is sufficiently less traumatic for the patient. I will tell you about a situation we often encounter in our clinical work: a baby with more than 1/3 of an area of hemangioma on the face or hands, and the family eagerly requests surgery as soon as he enters the hospital. Even if it is removed and repaired by skin grafting, who can guarantee that his appearance will not be damaged and the function of his hand will not be damaged. Today, when the child is still young and the parents help him to make such a medical decision, who can guarantee that the cosmetic defect and the lack of function will not bring more pain to his growth. The above example tells parents that surgery is the most effective, but it is not a cure-all. The treatment of hemangioma is a long process that requires patients, parents and doctors to work together for a long time to choose the right treatment plan according to the actual situation.
