Generally thyroid nodules are operated on in the following cases 1. when a malignant nodule is suspected, 2. when a single nodule is larger than 3 cm, and 3 when the nodule is large enough to affect the cosmetic appearance. In your case you can start with a fine needle puncture of the nodule and decide whether to operate based on the results of the puncture.
