The formula is a combination of cold and heat, with a wide range of combinations, so it has traditionally been cold for doctors, and even said that “Wu Mei pill, not pass the formula, not in the use of the formula, whether it is deficiency, is real, can not be the main also”, misleading readers. In clinical practice, especially in combination with the study of the theory of the five movements and six qi, and in accordance with Zhongjing’s theory of the six meridians to be solved, Wu Mei Wan can solve many clinical difficulties in the syncope of yin. It is not just a case of vomiting roundworm.” Since Umei Pill is the main formula for syncopal yin disease, in clinical application, we should fully grasp the syllabus of syncopal yin disease in the Treatise on Typhoid, closely follow the pathogenesis of “yin exhaustion and yang growth, cold and heat mingling”, and combine the provisions of syncopal yin when it wants to be resolved. This formula is of great value to the clinic.  1.1 As the ruling medicine of Wu Mei Pill, the dosage of Wu Mei Pill should be large. “Through long-term clinical application, we found that wu mei is flat and sour in nature, entering the liver meridian, the main peace of mind, to remove heat and fullness, as the main medicine of this formula, but the dosage is larger, only to be able to special effect macro. Clinical application mostly starts from 30g, and as much as 100g, often achieving unexpected results.  1.2 As a formula for both cold and heat, the clinical application of Wu Mei Wan must deal with the relationship between the proportion of cold and hot drugs. The clinical evidence is often through the patient’s symptoms, reasonable adjustment of the ratio of hot and cold drugs, cold, then increase the proportion of warming drugs, such as pepper, Sichuan pepper, cinnamon; hot, then heavy use of Huang Lian (heartburn and heat must be reused!) Case example 1, Guo, male, 60 years old, initial diagnosis on 07/19/2014 The patient had recurrent cough and asthma for more than 5 years, with aggravation for more than 10 days. History: 5 years ago, the patient developed a cough with a small amount of white mucous sputum, chest tightness and shortness of breath, and phlegm in the larynx without any obvious cause, and went to a local hospital, where he was treated with anti-infection and phlegm, but did not see any significant improvement. The cough and asthma were significantly reduced after taking Chinese herbal medicine to benefit the Qi and dispel wind, and to relieve the cough and asthma. He took “loratadine” but did not see any significant improvement, so he came to our outpatient clinic again. At the time of consultation, the patient had coughing fits, coughing when the throat itched, dry cough without sputum, chest tightness and wheezing, wheezing at night, mostly in the second half of the night, coughing very hard, Qi rushing up to the chest, heart and chest irritability, head dryness and sweating, thirst, nasal congestion and runny nose, no fever, abdominal distension, food and appetite still acceptable, stool still regulated, night sleep. The tongue is light red, with thin yellowish greasy coating and thin string pulse. On auscultation, a distinct croup can be heard in both lungs. The patient has a past history of “allergic rhinitis”, which has not been treated. TCM diagnosis: croup. The treatment is to reconcile yin and yang of the internal organs, with a mixture of cold and heat; the formula is based on harmonizing yin and yang, with the addition and subtraction of Wu Mei Wan. 15g, Dajiao 10g, Su Ye 10g. 4 doses, decoction in the usual way.  On July 23, 2014, the second diagnosis: after taking the medicine, the patient’s symptoms were slightly reduced, but the patient still had coughing and wheezing at night, dry cough without sputum, slightly dry mouth, irritable heat and sweating, light red tongue, thin yellowish greasy coating, thin string pulse. The original formula was supplemented with the following herbs: Wu Mei 60g, Xiao Xin 3g, Gui Zhi 10g, Radix et Rhizoma Pinelliae 3g, Huang Lian 8g, Huang Bai 15g, Radix Angelicae Sinensis 10g, Radix Codonopsis Pilosulae 15g, Chuan Jiao 4g, Dang Jiang 6g, Almond 10g, Hou Pu 15g, Gua Gua Piper 10g, Allium 10g, Fa Han Xia 10g, Scape Drabble 15g, Da Zao 10g, Su Ye 10g, Sang Bai Pi 15g. 2 doses. The patient complained that after taking the first dose of medicine, the cough and asthma were significantly reduced, and the heat in the heart and chest was not bothered. 3 doses of medicine were taken, and now the cough and asthma are not obvious, and all symptoms are lost. The original formula was continued with 5 doses to consolidate the effect.  The syllabus evidence of syncope in the Treatise on Typhoid is “Syncope is a disease of thirst, with Qi hitting the heart, pain and heat in the heart, hunger and lack of desire to eat”, which is quite consistent with the patient’s chest congestion and asthma, with severe asthma at night, mostly in the second half of the night. The disease involves the liver, heart and lung. The lesion involves the liver, heart and lung, resulting in the imbalance of yin and yang in the internal organs and Qi rebellion, coupled with alcohol consumption, touching “phlegm glued on the diaphragm and Qi congestion in the internal organs”. The main formula for treatment is Wu Mei Wan, which harmonizes yin and yang; although the cold symptoms are not obvious, the patient’s abdominal distension is sometimes made, and the loss of transportation in the middle jiao has already appeared. It is used to calm asthma and remove phlegm in the throat. In the second consultation, because the effect of the first consultation was not obvious, we added more evidence. Although the pathogenesis of this disease is “mixed cold and heat”, heat is more than cold, and as the Nei Jing says: “All rebellious rushing upward belong to fire”, so we increased the dosage of Huang Lian and Huang Bai. Shennong Ben Cao Jing” cloud: “plum, taste sour and flat. The patient took the medicine and the effect was like a rafter, which shows the accuracy of dialectic treatment.  2, Lu, female, 56 years old, initial diagnosis on June 6, 2014 The patient had pain and discomfort in the right hypochondrium for more than 10 years. At the time of consultation, the patient was suffering from pain in the right hypochondrium, preferring to lie on the right side. The first time from the usual method to dredge the liver and regulate the qi and luo method, the formula selected from Xiang Shen Xuan Fu Hua Tang, Si Wei San, Jin Ling Zi San, and Lost Smile San were all ineffective. 10g, Radix Codonopsis Pilosulae 12g, Radix Chuan Pepper 4g, Radix Ginger 5g. 7 doses, decoction in the usual way.  On June 13, 2014, at the second consultation, the patient complained of great reduction of right hypochondriac pain, dry mouth, and reduction of elimination of grains and good hunger. The original formula was increased to 35g with the addition of 10g of whole guavas, 5g of safflower and 5g of roasted licorice. 7 doses were taken with the usual decoction.  On June 20, 2014, at the third consultation, the patient complained of hypochondriac pain, a slightly dry throat, a burning sensation in the right hypochondrium, thin white fur, dark texture, and thin pulse. The effect was not changed and the original prescription was continued for 7 doses.  Note: “Ling Shu. The treatment is to reconcile yin and yang with Wu Mei Wan, which is sour in taste, and is paired with Dang Shen and Gan Cao to transform yin and soften the liver and relieve pain. At the second consultation, the patient’s hypochondriac pain improved significantly, and he complained of a burning sensation in the right hypochondrium, so he added the original prescription of Guahuosan, which is from the hand of Huang Gutan, a senior foot of Wang Ji, and is used for “hypochondriac pain due to dryness of liver qi”. “.
