OBJECTIVE: To investigate the efficacy of the modified Palomo procedure combined with embolization in the treatment of moderate to severe varicocele. METHODS: In group A, 32 patients with varicocele, aged 18-35 years, average 25.5 years, including 10 cases of II0 varicocele, 22 cases of III0 varicocele, 20 cases of pure left varicocele, and 12 cases of bilateral varicocele at the same time, all received the modified Palomo procedure, and after high ligation of the internal spermatic vein, one thicker internal vein on one side was selected for incision, and the stagnant blood in the varicocele was drained, and the distal end was treated with The distal end of the vein was rapidly injected with 10 ml of 50% dextrose, and the varicose vein was rapidly kneaded to allow the varicose vein lumen to fully adhere to the embolus, and then the incised vein was ligated. In the same period, 30 patients with varicocele in group B, aged 15-38 years, average 26.5 years, including 10 cases of II0 varicocele and 20 cases of III0 varicocele, 18 cases of pure left varicocele and 12 cases of simultaneous bilateral varicocele, received only the modified Palomo procedure. The patients were followed up from 12 months to 18 months, and the scrotal color Doppler was reviewed to understand the recurrence of varicocele and testicular blood flow, and the statistical differences in recurrence rates between the two groups were compared by chi-square test. There were 2 recurrences in group A, all of whom were patients with III0 varicocele, with a recurrence rate of 6.25% (2/32), and 9 recurrences in group B, including 3 cases of II0 varicocele and 6 cases of III0 varicocele, with a recurrence rate of 30% (9/30). the recurrence rate in groups A and B was determined by chi-square test with a P value <0.001< font="">, and the difference was statistically significant. Conclusion: 1, 50% glucose solution as embolization agent is safe and effective; 2, modified Palomo procedure combined with embolization is an effective method for the treatment of moderate to severe varicocele, with good recent results and low recurrence rate, especially for patients with III0 varicocele.
