A brief discussion of the TCM pathological mechanism of depression

The number of people suffering from anxiety disorders and depression has increased significantly.  Depression is defined in medical textbooks as an emotional disorder. Its most significant clinical manifestations are mood swings, reduced energy, reduced activity, and loss of pleasure and interest, and severe depression even leads to mental retardation and thinking difficulties. It seriously endangers the physical and mental health of people, and in serious cases, endangers the lives of people. The famous Hong Kong actor Leslie Cheung died young, and recently our young translator Sun Zhongxu committed suicide due to depression in Guangzhou at the age of 41. According to the World Health Organization, by 2020, depression will be the biggest cause of disability or death in adulthood, except for heart disease.  Depression does not discriminate between men, women and children. British Prime Minister Winston Churchill famously said during World War II: “Depression in my heart is like a black dog that bites me every chance it gets”. CCTV host Cui Yongyuan suffers from depression, and many other young students also suffer from this disease. Due to biological and external factors, women are twice as likely to get depression as men.  Many people in society think that there is no such thing as depression, that it is a small-mindedness and inability to think, but this is not the case. Many people have depressive tendencies before they suffer from depression and show abnormalities in their bodies and emotions, and this is the time to seek medical attention. When you have the following symptoms, you must see a doctor urgently, do not let the best time to treat.  First, pessimism and despair, depressed mood, loss of interest, unresponsiveness, disinterested in everything, and unhappy with everything. The second is paranoia, hallucinations, reluctance to meet people, reluctance to talk, aggression and easy to cry, fatigue, low self-esteem and self-blame, temperamental and uncontrollable. Third, stubborn insomnia, difficulty in sleeping, easy to wake up after sleeping, difficult to sleep after waking up, and even sleeplessness all night. Fourth, inexplicable fear and dread, panic and sweating, and fearfulness. Fifth, adolescents have difficulty in learning, lack of concentration, and most of their grades drop significantly. In the eyes of teachers and classmates are good and honest children, but at home stubborn, irritable, temper tantrums, go to extremes. The above symptoms are normal in all major hospitals for routine examination, without any organic lesions, but the patient is indeed uncomfortable and ineffective after multiple treatments.  How does TCM recognize this disease from its own theory? As you know, the most basic feature of TCM is the holistic view of the body, the correspondence between heaven and man, the coordination of yin and yang, and the balance of the internal organs. Through the above symptoms, according to the basic theory of TCM, briefly analyze the mechanism of the appearance of the above symptoms.  Liver and gallbladder disorders The liver and gallbladder belong to the eastern wood and are related to each other. The liver is the main organ for blood collection and strategy, and in the will for anger, while the gall bladder is the main organ for rising and deciding. Liver is yin and bile is yang. If yin and yang are out of balance, liver is strong and bile is weak, so there is more strategy but less determination, resulting in rage and bile cannot be restrained. Therefore, the patient is usually timid and cautious, but cannot control his fire.  Reverse transmission of lung heat In the treatise on warmth and fever, the therapist Ye Tianshi suggests that “warm evil is received upward, first offends the lungs, and passes backward to the pericardium.” Depression is also the result of the heat of the lung jin prevailing, which insults the heart fire, resulting in insomnia, irritability, and palpitations in the patient. Clinically, we find that many patients tell that they are often on fire, with dry throat and thirst, and constipation. After a long time, insomnia, fatigue, irritability and other mental problems appear. This lung heat is the main cause, as the lung resides in the upper jiao, and fire in the upper jiao is not treated and appears to be transmitted back to the pericardium.  Lung-heat and kidney deficiency The phenomenon of lung-heat is caused by improper diet, irregular living, or external sensation into the lungs, or long-standing failure to treat. The kidney deficiency can be caused by congenital deficiency or prolonged illness, or by frequent intercourse or overexertion. However, the lung is too strong to produce water (kidney), which can also cause or aggravate kidney deficiency. Lung Qi is strong, and can be divided into fire and Qi. Fire is the fire, as already mentioned, and Qi is the manifestation of chest tightness, chest fullness, Qi rebellion, coughing, asthma attacks, difficult to get down although the stool is not hard, “Inner Path”: Qi has a surplus is fire. Therefore, fire and Qi are not contradictory, but only in which direction. Because the lung is too strong to produce water, the kidney yin is insufficient and the kidney yang is also deficient, resulting in upper strength and lower deficiency or upper heat and lower cold. Because of kidney deficiency, water does not produce wood, making bile qi even more deficient. The condition intensifies, so the patient develops unexplained fear and dread. Those suffering from depression are more timid and easily stressed.  Kidney-Yang deficiency
Kidney is divided into kidney yang and kidney yin,] kidney yang is also known as true fire, the fire of life. It is the driving force of the body’s functions, and a deficiency in kidney yang is a rootless fire. Therefore, some people have dry throat and pain, dry mouth and tongue, sores on the lips and tongue, but also back pain and knee weakness, tinnitus, vertigo, fear of cold, tiredness and fatigue. This pathology is often an important cause of restlessness. Treatment should not only clear heat and remove fire, but also induce fire to return to its source, which is the fundamental reason for the difficulty of treatment.  Conclusion Depression is not an incurable disease, but why is it treated by many people with little success. The treatment must be based on the root of the disease, which must be treated from the root, that is, from the overall yin and yang conditioning, the balance of the internal organs. You cannot treat the headache and the foot, only pay attention to the local and lose the whole. People suffering from depression or depressive tendencies, in addition to depressive symptoms, whether there are other discomfort, after a comprehensive dialectic, to find the root of the disease, comprehensive treatment, will receive satisfactory results.
