In vitro fertilization embryo transfer treatment, commonly referred to as IVF treatment, is more visual and generic. The ovulation protocol before egg retrieval is particularly important and is the basis for the success of IVF treatment. First, the long protocol, as the name implies, takes longer. Generally speaking, the long protocol is used in young patients with normal ovarian function. Ovulation monitoring is performed for regular menstrual periods, and the treatment is started 1 week after ovulation, and ovulation promotion therapy is started after the ovaries reach the standard of down regulation. The advantage is good synchronization of follicular development. For patients with polycystic ovary syndrome or ovulation disorders, oral contraceptive artificial cycle therapy is started on the 5th day of menstruation and descending regulation is started on the 21st day. The duration of down regulation and ovulation promotion usually takes about 25 days. Second, short regimen or ultra-short regimen for older patients with slightly lower ovarian function. Treatment is usually started on the 2nd day of menstruation with subcutaneous injection of GnRH-a 0.1mg, and the next day the dose of ovulation-promoting drugs is decided according to the hormonal situation, and the general ovulation-promoting time is about 8-10 days. Nowadays, patients in our center mostly use the ultra-short protocol, and GnRH-a is usually applied for 3-5 days. Thirdly, the ultra-long protocol, which is applied to patients with adenomyosis and ovarian coeliac. The first dose is usually 1.8mg/3.75mg, and after 28 days, the decision of additional Dafirin treatment is made according to the hormonal and ultrasound conditions. After 28 days, additional Dafylline treatment will be administered according to the hormonal and ultrasound conditions. In patients with coeliac disease, coeliac puncture may be required. Fourth, natural cycle or microstimulation regimen for patients with declining ovarian function, older age and low response. The timing of egg harvesting is based on follicular growth and hormonal profile without the use of any ovulation-promoting drugs, called natural cycle. The use of small amounts of urotropin, clomiphene or letrozole to promote ovulation is called microstimulation. The advantage is the low cost of medication and the low financial burden to the patient, but the disadvantage is the high rate of cycle cancellation. Above, the common protocols used in the IVF treatment process are briefly introduced, but the protocols are not set in stone, but should serve the person, i.e. what is suitable is the best, and individualized medication is the hard truth!
