Bleeding is a common symptom of acute leukemia, bleeding sites can spread throughout the body, the skin gums nasal bleeding is the most common, there can also be retinal intra-ear bleeding and intracranial gastrointestinal respiratory tract and other visceral hemorrhage, excessive menstruation is also more common in women, and can be the first symptom. M3 and M5 subtypes of AML bleeding is more serious, especially M3 patients are prone to complications of disseminated intravascular coagulation (DIC) and intracranial bleeding and death. The bleeding is more severe in the M3 and M5 subtypes of AML. What should be done if bleeding occurs in acute leukemia? (a) Treatment of the cause of the acquired bleeding disorder must be aggressively addressed in order to achieve the goal of treatment. This includes gene therapy, coagulation factor supplementation, and surgical hemostasis. (ii) Hemostasis problems must be targeted and selected to avoid abuse of hemostasis*, broadly divided into vascular, platelet bleeding then apply compression hemostasis and improve vascular permeability* substances; immunosuppressive supplementation platelets and other coagulation factors deficiency, alternative supplementation hyperfibrinolysis then antifibrinolysis, etc. The methods to prevent bleeding in acute leukemia patients are: (1) maintain skin integrity: ① avoid activities that may cause physical injury, (2) avoid trimming nails with steel plates or fine mesh contouring knives and shaving with electric beard knives, (3) avoid wearing tight clothing, rough textiles and using tourniquets. Maintain the integrity of the oral mucosa: (1) eat softly blended foods and avoid foods that cause temperature, mechanical and chemical irritation; (2) use a soft-bristled toothbrush to gently clean the mouth, rinse with a low-alcohol mouthwash, avoid flossing, and use petroleum jelly to lubricate the lips. Maintain the integrity of the mucosa of the gastrointestinal tract: ① promote water intake and increase activity to avoid constipation; ② prevent constipation by giving stool softeners or Chinese* tonics in the usual way; ③ avoid rectal invasion of human treatment measures, such as the use of enemas, suppositories and anal thermometer; ④ avoid gastrointestinal irritation causing gastric bleeding, and if steroids must be taken, take preparations or dairy products at the same time. Maintain the integrity of the mucosa of the upper respiratory tract: ① keep the nasal mucosa moist; ② increase the humidity of the air and maintain the humidity in the home at 40%; ③ sit up immediately in case of rhinorrhea and apply pressure to the nostril area under the bridge of the nose. Maintain the integrity of the genitourinary tract mucosa: (1) ingest more than 3000 ml of water daily: (2) avoid vaginal irrigation and vaginal suppositories; (3) instruct the use of water-soluble lubricants before sexual intercourse; (4) use female hormones under medical supervision if necessary to prevent menstrual flow. Avoid elevated cranial pressure: avoid forceful breath holding and vigorous activity when platelets are <20x109/L.
