Why do hepatitis B patients need treatment? It was reported that Mr. Chen, after suffering from hepatitis B, kept it secret for fear of discrimination; he also thought that it could not be cured anyway and did not undergo treatment. He developed liver cancer a few years later and died not long ago. At that time, the liver disease doctors all felt sorry for him. Experts from Chongqing Liver Disease Treatment Center: The importance of treatment for hepatitis B patients needs to be considered from two aspects. First, as far as the patients themselves are concerned, it is a consensus in the medical field that chronic hepatitis B will further develop into cirrhosis, hepatocellular carcinoma or liver failure if it is not treated actively and effectively. According to research data, the number of people infected with hepatitis B worldwide has exceeded 350 million, of which 15% to 40% will develop into cirrhosis, liver failure or liver cancer. Han Qingrong, Department of Infectious Diseases, Zhangqiu People’s Hospital Cirrhosis: High-risk factors for the development of cirrhosis include high viral load (HBVDNA), persistent positivity for Hepatitis B e antigen (HBeAg), and high or repeated fluctuations in alanine aminotransferase levels. The incidence of cirrhosis is higher in HBeAg-positive patients (commonly known as “triple positive”) than in HBeAg-negative patients. Among chronic hepatitis B patients with a disease duration of more than 5 years, 12%~25% will develop cirrhosis. Hepatocellular carcinoma: Hepatitis B surface antigen (HBsAg) positive patients with chronic hepatitis B are at least 100 times more likely to develop hepatocellular carcinoma than the general population, with the highest incidence in patients with chronic hepatitis B complicated by cirrhosis, and the incidence of hepatocellular carcinoma in cirrhotic patients with a duration of more than 5 years is 6%~15%. The mortality rate of liver cancer is known to be extremely high. Liver failure: the chance of cirrhosis patients with a disease duration of more than 5 years to develop liver failure is 20%~23%. The alarming figures are: statistics show that 15%~25% of Chinese chronic hepatitis B virus-infected people will eventually die of hepatitis B-related diseases, and 330,000 people die of hepatitis B-related diseases every year, accounting for the first place of infectious disease deaths in China. Secondly, as we all know, hepatitis B is a contagious disease, and its contagiousness depends mainly on the level of HBVDNA in the blood, and has nothing to do with the level of serum alanine aminotransferase, aspartate aminotransferase or bilirubin. Therefore, aggressive treatment at the right time to minimize HBVDNA levels quickly can reduce the infectiousness of the disease, help patients communicate better and improve their quality of life. It can be seen that high levels of HBVDNA and HBeAg positivity are the most critical factors leading to liver cirrhosis and liver cancer, so actively treating hepatitis B, reducing HBV DNA levels and achieving HBeAg seroconversion are extremely important in delaying and preventing the development of liver cirrhosis and liver cancer. At the same time, reducing the infectiousness of the disease by lowering the HBV DNA level will help patients establish harmonious interpersonal communication and improve the quality of life. Therefore, the treatment should also choose drugs that can quickly and effectively reduce the level of HBV DNA and have a high HBeAg seroconversion rate.
