Post gallbladder syndrome medical case

Ma Moumou, male, 72 years old, Lanzhou City Initial diagnosis (November 10, 2010): The patient has been post-cholecystectomy for more than 5 months. In recent January, without any obvious reason, he developed distention and fullness in the abdomen, bitterness in the mouth, dullness, lack of food and drink, weakness, dry stools, one line for two days, unpleasant, and urine regulation. Ultrasound examination of the abdomen was performed at Landa Hospital, which indicated that “the gallbladder had been removed”; blood count showed that it was normal. He was considered to have post-cholecystectomy syndrome and was treated with “multi-enzyme tablets”, which was not effective. His tongue is pale red, with fat body and tooth marks on the side, white and slightly yellowish greasy coating, left pulse is stringent and slippery, and right Guan pulse is sunken and thin. This is a case of disharmony between the gall bladder and stomach, and Qi is not flowing smoothly. This formula is based on Xiao Chai Hu Tang plus and minus. Prescription: Radix Bupleurum 12g Scutellariae 12g Radix Scutellariae 10g Pericarpium Citrus 10g Radix Codonopsis 12g Radix Glycyrrhiza Glabra 10g Magnolia 15g Radix Chrysanthemum 20g Fructus Mucuna Pruriens 10g Poria 10g Fructus Atractylodes 10g Dandelion 15g Radix et Rhizoma Gastrodiae 15g Rhizoma Yam 15g Frying Maltan 10g Decoction of 500ml twice a day after meals. 4 doses. Second consultation (November 17): abdominal distension is reduced, stool is soft and not smooth, mouth bitterness is obviously reduced, food intake is enhanced, tongue is light, body is fat, moss is white and greasy, pulse is smooth. This phase fire of the bile has been removed, dampness appears, so the above add products to strengthen the spleen and remove dampness. Prescription: Bupleurum 12g Scutellaria 12g Radix Scutellariae 10g Chen Pi 10g Radix Codonopsis 12g Poria 10g Yam 15g Fried Atractylodes 10g Raw Job’s Tear 20g Chicken Neijin 10g Fried Wheat Germ 10g Mu Xiang (after the next) 10g Dandelion 10g Ginger 8g Citrus aurantium 10g Followed by 7 doses and healed. Note: The patient is suffering from post-biliary cyst syndrome, which belongs to the category of “fullness” in Chinese medicine. If the liver and gallbladder are unfavorable, the bile is not drained, and the bile goes up to the stomach, then the mouth is bitter; if the stomach qi does not descend, the abdomen is distended; if the spleen is not healthy, dampness and phlegm are produced, then the stomach is sluggish, not thinking about eating and drinking, and the moss is greasy. The sunken pulse on the right side is a sign of spleen deficiency, while the slippery pulse on the left side is a sign of hyperactive liver-biliary fire. Therefore, the best treatment is to take conciliatory agents, strengthen the spleen and stomach, dredge the liver and gallbladder, and regulate the qi flow, and treat both deficiency and reality.
