Depression Self-Rating Scale (SDS) Note: There are twenty texts below, please read each one carefully to understand the meaning, and then cross in the appropriate box according to your actual situation in the recent week, after each text there are four boxes, indicating: A no or little time; B small part of the time; C quite a lot of time; D most or all of the time. 1.I feel sullen and depressed; 2.I feel the best in the morning; 3.I cry or feel like crying for a while; 4.I don’t sleep well at night; 5.I eat as much as usual; 6.I feel as happy as ever when I am in close contact with the opposite sex; 7.I find that I am losing weight; 8.I have the pain of constipation; 9.My heart beats faster than usual; 10.I feel tired for no reason I feel tired; 11, my mind is as clear as usual; 12, I feel no difficulty in doing what I often do; 13, I feel restless and cannot calm down; 14, I have hope for the future; 15, I get angry and excited easily than usual; 16, I feel that it is easy to make decisions; 17, I feel that I am a useful person and that someone needs me; 18, I have an interesting life. 19.I think if I die others will live better; 20.I am still interested in the same things I am usually interested in. Scoring: Forward scoring questions A, B, C and D are scored as 1, 2, 3 and 4; reverse scoring questions are scored as 4, 3, 2 and 1. Reverse scoring questions number: 2, 5, 6, 11, 12, 14, 16, 17, 18, 20. The total score multiplied by 1.25 to take the whole number, that is, the standard score, the smaller the score, the better the cut-off value of 50.
