What are the indications for invasive cholecystolithotomy?

  Gallbladder stones and gallbladder polyps are common diseases. Minimally invasive laparoscopic surgery has been skillfully carried out in our hospital. The gallbladder is a useful human organ, and while laparoscopic cholecystectomy is being performed, endoscopic minimally invasive biliary stone extraction is gradually being performed and is being recognized by more and more surgeons. In December 2007, the first national academic conference on endoscopic minimally invasive biliary preservation was held, especially in 2008, the 13th national academic conference on biliary surgery, China’s biliary surgery masters clearly pointed out: endoscopic biliary stone extraction (development) is a major event in the 21st century, is a major event in China! The specification of endoscopic biliary lithotripsy has been incorporated into the textbook of higher education institutions – the national textbook of surgery of pharmacy department. So are all patients with gallstones eligible for biliary stone extraction surgery?  Only if the patient still has a functioning gallbladder can biliary stone extraction surgery be performed. For patients whose gallbladder is no longer functional, gallbladder removal surgery should be recommended without hesitation. Specifically, what are the indications for gallstone extraction?  Indications for gallbladder removal: 1, no separation in the gallbladder, good acoustic transmission in the gallbladder, single or multiple stones.  2, no history of acute inflammatory upper abdominal open surgery or perforation.  3, no obvious symptoms of gallbladder stones, which is usually referred to as “quiet stones”.  4. The gallbladder should not be too large or too small. The gallbladder must have a good contraction function, and the degree of contraction should be more than one-third.  5. The gallbladder is not atrophied.  6. The age should not be too old.  Patients must have a complete preoperative examination, a clear diagnosis, and be sure that the indications for gallstone extraction are met and there are no contraindications to surgery before treatment is done.  It is advisable to treat gallstones early, otherwise, waiting until there are symptoms will not only be painful, but surgery will also bring more danger. Moreover, gallstones can often block the bile ducts, causing biliary colic, jaundice, perforation, nausea and vomiting, blood pressure drop, irritability, biliary bleeding, shock coma, pancreatitis, liver cirrhosis, liver abscess, liver failure and liver and kidney syndrome, as the so-called pancreatic bile is difficult to separate, liver and gallbladder. In more serious cases, gallbladder cancer may be induced, although the proportion is not high, gallstones are an important cause of gallbladder cancer.  Therefore, it is recommended that patients with gallstones should be treated as early as possible based on the treatment principle of “early treatment and prevention of changes” to avoid leaving the root of the problem.
