As a hepatobiliary surgeon, I find that many of the patients seen in the clinic are troubled by gallbladder stones. So, what should I do if I have gallbladder stones? 1.If gallbladder stones are found in the usual checkup, usually without any symptoms, and no thickening of the cyst wall or coarse mucosal echogenicity of the gallbladder or reduction of the gallbladder volume is found in the imaging examination such as ultrasound, you can observe and follow up regularly. 2.If gallbladder stones are found because of abdominal discomfort, such as abdominal pain, abdominal distension, abdominal discomfort after eating and drinking, etc.; and the above symptoms obviously affect the patient’s normal work and life, surgical treatment can be considered. 3.If the patient is >50 years old and gallbladder stones are found, or combined with more serious heart disease, diabetes, or other serious medical disorders, surgery can be considered. 4. If gallbladder stones are found with more serious inflammatory manifestations, such as severe epigastric pain, fever, nausea and vomiting, and no significant effect of medical treatment. If the onset of the disease does not exceed 72 hours, the patient should be seen in surgery for timely surgical treatment. However, it should be noted that conservative management should be considered if the onset exceeds 72 hours. Ultrasound localized gallbladder puncture and drainage can be performed. Consider surgical treatment after the patient’s symptoms and signs are significantly relieved. 5. Another situation is that if gallbladder stones are suspected to be combined with tumors, whether malignant or benign, if they are combined with gallbladder polyps, or gallbladder adenoma, or gallbladder cancer, they should be seen by the hepatobiliary surgery department for timely surgical treatment. 6. Specially, for patients with gallbladder stones, please do not believe in the so-called non-operative treatment methods such as laser treatment. My personal advice is that if you do not want to have surgery, then do not consider other treatments. However, if there are acute symptoms of infection, anti-infection treatment should be given promptly by infusion, and if necessary, surgical treatment should be given promptly.
