Primary liver cancer is one of the most common malignant tumors in China, which is characterized by late detection, rapid development, difficult treatment and poor prognosis, and is called the “king of cancer”. The recurrence rate of small hepatocellular carcinoma is also as high as 40-50%. Many patients have diffuse hepatocellular carcinoma, or ascites, or renal insufficiency, so it is difficult for them to receive surgery, hepatic artery embolization, chemotherapy and cryotherapy, etc. Chinese medicine has become the main treatment method. In recent years, the treatment of primary hepatocellular carcinoma with the method of nourishing Yin, benefiting Qi and activating blood to fight cancer has achieved better efficacy, and the experience of Chinese medicine in treating hepatocellular carcinoma is summarized as follows for the reference of our colleagues. According to its clinical manifestations and characteristics, liver cancer can be classified as “duress”, “abdominal pain”, “jaundice”, “accumulation” and “obstruction” in Chinese medicine. “jaundice”, “accumulation”, “obstruction” and so on. The pathogenesis is complex, and it is difficult to see a single pathogenesis clinically, and it is mostly manifested clinically as a mixture of deficiency and reality, cold and heat. The main pathological feature is the deficiency of positive Qi, that is, “if the positive Qi is deficient, it becomes a rock”. Zheng deficiency mainly refers to deficiency of liver qi and dysfunction. The evil real refers to the accumulation of phlegm and dampness, stasis and toxins. Treatment experience: when the disease reaches liver cancer, the positive energy is leaving, phlegm-dampness and stagnant toxin are accumulating, and the positive deficiency and evil are real, so the treatment should not only eliminate the evil but also help the positive. However, the key of Chinese medicine treatment for liver cancer is not only to eliminate the cancer itself (eliminating evil), but also to restore the physiological function of liver under the guidance of holistic concept. Only by treating from liver deficiency (supporting righteousness) can the development of liver cancer be stopped and some of the tumors disappear, that is, when the righteousness is restored (function is restored), the evil will go away. There are two main types of deficiency of positive qi, namely liver yin deficiency and liver and spleen deficiency, and in either type, there are clinical signs of liver yin and blood deficiency. Clinical diagnosis and treatment should grasp the main evidence, check the tongue and pulse, liver yin and blood deficiency is seen in dry mouth, red tongue with little coating, thin pulse strings or the pulse is long and short with yin and blood deficiency and liver depression, blood deficiency is seen in excessive dreaming. Deficiency of Liver and Spleen is seen in diarrhea, poor appetite, thin pulse or pulse tendency coming and going with the evidence of deficiency of Liver-Yin and Blood and Liver-depression. The evil realities are phlegm and dampness, stasis and toxin accumulation, which are the etiology and pathological products of liver deficiency. Therefore, the treatment of liver cancer is not only to nourish Yin and Blood, nourish Spleen Qi, regulate Qi and invigorate Blood, attack toxins with toxins, facilitate diuresis and clear heat, etc., which are combined to fight cancer on the basis of improving the internal environment. 1. Support: Mainly nourishing Yin and Blood and replenishing Spleen Qi: Liver harbors Blood and is the main drainage agent, with Yin and Yang in the body. For qi management, it is better to use fruit peels and flowers that do not hurt the stomach, consume qi or hurt yin. Some patients with hepatocellular carcinoma do not suffer from any clinical discomfort, so the above drugs should be used sparingly and in light dosage. The above drugs should be used sparingly and in lighter dosage. The drugs of “promoting qi and blood circulation” can help the liver and promote the recovery of liver draining function; when liver draining function is recovered, phlegm-dampness and stagnant toxins should be removed, and phlegm-dampness and stagnant toxins are less likely to accumulate. (2) Attacking toxins with toxins and clearing heat: Although the root cause of tumor formation lies in the lack of consolidation of righteousness, the formation of lumps must be embedded with poisonous evil, and in the treatment process, it is difficult to be effective only with drugs that support righteousness and promote blood circulation. The class of drugs are toad skin, gecko, spotted fungus, longan, centipede, small white flower snake, etc.. The medicine of “attacking poison with poison” mostly hurts the gas and stomach, so those who have no tumor and are weak after surgery should use less medicine, the dosage should be light and the duration of medicine should be short; those with rapid tumor development and strong body can choose 2-3 kinds of medicine and the amount can be large. According to traditional medicine, “under the firmness of the tumor, there must be a volatile yang”, so while attacking the tumor with poison, we can add the products to clear the heat, such as dandelion, tiger scepter, bupleurum, wild chrysanthemum and raw earth, etc. According to the degree of heat, 1-2 kinds of drugs can be chosen, but they should not be used as the main medicine to treat liver cancer, otherwise the tumor will not be eliminated but the stomach gas will be hurt. (3) Facial diuresis: Patients with liver cancer mostly have malignant effusion, short urine, swelling of face and limbs, so it is appropriate to use diuretic drugs, such as Fu Ling, Money Grass, Sang Bai Pi, Big Belly Skin, Two Ugly, Pig Fu Ling, etc.; stomach umbilical pressure, constipation or poor stool, persistent food, bloating and pain in the abdomen, pressure pain in the ribs, tight or sunken pulse, it is appropriate to use laxative drugs, such as rhubarb, Yuan Ming Powder, betel nut, tomato leaf, lycopodium, etc. This is the common method. However, attacking the evil in the intestines and stomach can also smooth the qi flow, mediating the middle state, so that liver qi can be relieved, blood stasis and phlegm accumulation can be removed, and the laxative can also dispel the tangible evil qi to remove cancer toxicity and prevent poisoning by accumulation of toxic herbs, so even if there is no laxative symptom, the laxative can also be used, but with lighter and fewer drugs or only one of the methods is chosen. When applying attacking drugs, it is necessary to clarify the situation of deficiency and real evil, and to master the priority of attacking and helping. In conclusion, the treatment of liver cancer should mainly focus on nourishing Yin and Blood, tonifying Yang Qi, with the addition of Qi-regulating and Blood-activating drugs, attacking toxins with toxins, clearing heat and laxative.
