Combined Chinese and Western medicine treatment of intractable cirrhotic ascites

  The pathological mechanism of liver cirrhosis ascites is characterized by deficiency of Qi and deficiency of Yin, while the symptom of deficiency of Qi, water and blood stasis, but “Qi deficiency and blood stasis” is the basic pathological mechanism or symptom type. On the basis of the common basic evidence type of “qi deficiency and blood stasis”, the following main evidence types can be classified according to different individual conditions and stages of lesions of patients. It is necessary to pay attention to the basic treatment method of benefiting Qi and activating Blood according to the basic type of symptoms, and then add the main types of treatment methods, and add or subtract medicines appropriately.  Basic treatment method. Tonify the middle and benefit the qi, invigorate the blood and eliminate blood stasis. Formula: Si Jun Zi Tang combined with Peach Kernel Cheng Qi Tang, or Tonic Yang Returning Five Tang with addition and reduction. Ginseng 9g, Atractylodes macrocephala 12g, Poria 15g, Glycyrrhiza glabra 6g, Peach kernel 6g, Radix et Rhizoma Rheum 9g, Osmanthus 9g, Mangosteen 12g, etc.  The main types of evidence and treatment are: 1. Qi stagnation and dampness obstruction. Main symptoms: abdominal distension with no firmness when pressed, dysbiosis or pain, dullness with little food, distension after eating, belching, slightly reduced yagging, thin white and greasy tongue coating, string pulse. Edema of the lower limbs and short urine. Treatment: Drain the liver and regulate qi, move dampness and disperse fullness. Formula: Chai Hu Dredging the Liver with Stomach Ling Tang. Radix Bupleurum Chinense 9g, Radix Aromaticus 9g, Rhizoma Yujin 9g, Green Peel 9g, Rhizoma Chuanxiong 9g, Radix Paeoniae Alba 15g, Atractylodes Macrocephalae 9g, Atractylodes Macrocephalae 12g, Thicket 9g, Poria 15g, Poria 12g, Chen Pi 9g, etc.  2. Dampness and heat accumulation evidence. Symptoms: abdominal fullness, distension of the stomach and abdomen, irritability, bitterness, thirst, or yellowing of the face and skin, red urine, constipation or loose stools, red tongue, yellowish greasy moss or grayish black, and string pulse. Treatment: Clearing heat and relieving dampness, attacking and expelling water. Prescription: Zhongmanfenxiao Wan combined with Yin Chen Artemisia Tang. Hou Pu 9g, Hovenia 9g, Jiang Huang 9g, Scutellaria 9g, Huang Lian 3g, Gan Jiang 9g, Han Xia 9g, Zhi Mu 12g, Ze Di 9g, Fu Ling 15g, Pig Ling 12g, Bai Zhu 12g, Chen Pi 9g, Sha Ren 6g, etc.  3.Spleen and kidney yang deficiency evidence. Abdominal distension and fullness, shaped like a frog’s belly, wide at night, pale yellow, or indistinctly white, stuffy and dull, loose stools, cold in the extremities, swelling, unfavorable urination, fat tongue, purple texture, light white moss, sunken and feeble pulse. Treatment: Warming the spleen and kidney, promoting qi and promoting water circulation. Radix Rehmanniae Sinensis Pill with Wu Ling San. The formula includes 9g of Radix Aconiti, 9g of ginger, 9g of ginseng, 12g of Atractylodes Macrocephala, 12g of poria, 15g of poria, 9g of zedoary, 6g of roasted cinnamon stick, etc.  4. Liver and kidney yin deficiency evidence. Abdominal distension and fullness, or see blue veins exposed, obscure face, purple lips, dry mouth, sleeplessness, epistaxis, bleeding gums, short urine, red and vivid tongue with little fluid, little moss or light flaking, thin pulse. Treatment: Nourishing the liver and kidney, cooling the blood and resolving blood stasis. Recipe: Combining Consistent Decoction with Diaphragmatic Expulsion of Blood Stasis Soup, plus reduction. Bei Sha Shen 9g, Mai Dong 12g, Sheng Di 15g, Angelica Sinensis 12g, Fructus Lycii 12g, Chuan Xiong 9g, Mudan Pi 12g, Red Peony 12g, Wu Yao 9g, Wu Ling Li 9g, Tao Ren 6g, Safflower 6g, Xiang Shen 9g, etc.  2.Chinese medicine prescription for cirrhotic ascites Guan Youbo’s basic formula: 50g of Astragalus membranaceus, 10g of Angelica sinensis, 10g of Rhizoma Atractylodis Macrocephalae, 30g of Radix et Rhizoma Umbelliflorum, 10g of Almond, 10g of Orange Red, 30g of Poria, 15g of Radix Paeoniae Alba, 15g of Radix Paeoniae Alba, 20g of Zelenia, 10g of Radix Aromaticae, 10g of Lotus Root, 15g of Plantago ovata, 10g of Papaya, 15g of Magnolia officinalis, 3g of Ginger, 10g of Dahurian Bark, 15g of Salviae Miltiorrhiza. It has good effect in treating Yin deficiency and Blood heat, Qi deficiency and Blood stagnation in liver cirrhosis ascites.  Recipe 1: 30-60g each of Astragalus membranaceus and Atractylodes macrocephala, 30g each of black soybean and foxglove, decoction to be taken orally, 1 dose daily, divided into morning and evening. It is used for those with heavy ascites of liver cirrhosis, deficiency of middle energy, weakness of spleen and stomach, and inversion of albumin ratio.  Recipe 2: Panax notoginseng, turtle shell, black soybean, Chen gourd and asparagus, each in appropriate amount, in decoction, 3 times/d, for those with reduced albumin, inverted albumin ratio and obvious ascites.  The method of finishing down and expelling water. This method has been more reported in the past and is considered to reduce ascites by diarrhea. This is a method of expelling water, which should be applied with caution. 5-6 times /d of diarrhea is appropriate, and the water will stop when most of it is removed, and hepatic encephalopathy, electrolyte disorders, and upper gastrointestinal bleeding are contraindications.  3.Chinese medicine applied to the umbilical cord is one of the unique external treatment methods of Chinese medicine, as early as in the Spring and Autumn and Warring States, there is a record about the belly button application of medicine; to the Qing Dynasty, external treatment experts Wu Shiji established “ten dropsy to get water cream”, etc., to external application to expel water, so far still used for persistent cirrhosis ascites. In recent years, Jiang Chunhua used Gansui and Mannichol; Xia Dexin used Gansui, Cinnamon and Plantago; Lei Ling used Gansui, Altair, Fangqi and Shenxiang to compress the umbilical cord, and Tong Guangdong used Musk Huang Paste (30g of snail meat, 1g of musk, 1g of artificial cowberry and 2 roots of green onion) to compress the umbilical cord at the same time of basic treatment for cirrhotic ascites, and achieved good results.  According to the different characteristics of deficiency and actual symptoms of cirrhotic ascites and considering the common pathological mechanism of “obstruction of liver ligaments”, we formulated the formula for deficiency distension: benefiting qi and warming yang, promoting ligaments and expelling water, using raw astragalus, prepared sophora, cinnamon and clove, etc.; the formula for actual distension: promoting qi and removing stasis, promoting ligaments and expelling water, using raw rhubarb, lycopodium, incense and clove, etc. The effective parts and components were extracted and made into a babu agent, which was applied externally to the umbilicus. Patients in the test group showed significant reduction in abdominal distension, while the degree of ascites reduction was better than that in the control group, the hospital stay was shorter than that in the control group, and no toxic side effects have been found. At present, the formula has been fixed and named as “anti-distension ointment”, which has been clinically applied as a new technology and is being tested as a new clinical drug.
