Allergic purpura is a syndrome characterized by skin purpura, hemorrhagic gastroenteritis, arthritis and kidney damage, a kind of immune-related systemic small vasculitis, and the kidney damage caused by allergic purpura is called allergic purpura nephritis. The incidence of purpura nephritis accounts for 20~100% of allergic purpura, the incidence of foreign diagnostic criteria by kidney biopsy is up to 90% or more, can occur in any age group, mostly in children and adolescents, the highest in 6~13 years old, male to female ratio 3:1, good in the cold season. Allergic purpura belongs to the category of “hematuria”, “epistaxis”, “edema” and “paralysis” in Chinese medicine. In Chinese medicine, it is believed that allergic purpura nephritis is due to the internal heat of blood, external wind evil or excessive consumption of dry and hot meat and fishy products or drug allergy, resulting in wind and heat, the evil toxin becomes heat and disturbs the blood, forcing the blood to travel, overflowing the muscle skin and forming red spots, internal penetration into the interior, forcing the stomach and intestines, the middle jiao and frequent abdominal pain, blood in the stool. Internal invasion of the kidney, Yin deficiency and fire, disturbing the kidney channels, leads to blood in the urine and cloudy urine. The blood and qi circulation is not smooth, siltation in the joints and veins cause joint pain, very few patients with allergic purpura nephritis will appear spleen and kidney deficiency, turbid evil within the stop can lead to urinary toxicity serious illness. Clinical manifestations 1. extra-renal symptoms: rash, joint symptoms, gastrointestinal symptoms, as well as lymph node enlargement and hepatosplenomegaly, the rash is mostly on the dorsal side of the distal extremities; 2. extra-renal symptoms: hematuria mostly occurs within 4-8 weeks, a few months later, clinical manifestations of microscopic hematuria, intermittent hematuria, individual with proteinuria (mild) or even massive proteinuria course nephrotic syndrome, may have hypertension, edema. Very few patients have acute renal failure, and some patients have normal urinalysis but only abnormal renal function. Chinese medicine dialectical treatment Chinese medicine dialectical treatment is mostly based on the five points of wind, heat, toxicity, stasis and deficiency, in order to support and stop the bleeding. 1, wind-heat invasion Main symptoms: sudden onset of skin purpura, fever, sore throat or joint pain, abdominal pain, blood in urine, red tongue, thin yellow coating. Treatment: Expel wind and disperse evil, cool the blood to stop bleeding. Fu Ben stopping bleeding soup with Fang Feng, Scutellaria baicalensis, Atractylodes macrocephala, White Sclerotium, Peony peony bark, etc. 2, hyperthermia (acute attack) Main symptoms: purpura is red in color, dense distribution, dark red urine, red tongue with yellow coating, fine pulse. Treatment: Clearing heat and eliminating evil, resolving purpura and stopping bleeding. Fu Ben stopping bleeding soup with water hyacinth (first decoction), danpi, double flower, forsythia, yuan sen, huang lian, light bamboo leaf, small thistle, ground elm, white foxglove, etc. 3.Kidney deficiency and blood heat (recovery period) Evidence: purpura has receded, hematuria, dizziness and tinnitus, red tongue, thin yellow coating and fine pulse. Treatment: Nourishing Yin, tonifying the kidney, clearing heat and cooling blood. Fu Ben stopping bleeding soup with Cornu Cervi Pantotrichum, Sheng Di Huang, Poria, Yam, Radix et Rhizoma Polygonati, Dry Lotus Grass, Cyperus, Phellodendron, etc. 4. Lung and spleen qi deficiency (recovery period) Main symptoms: purpura is scattered, purpura is dark, fatigue, urine is red, protein may be present in urine, light coating, tender color, tooth marks on the edge. Treatment: Fu Ben stop bleeding soup with Atractylodes, Radix Codonopsis, Glycyrrhiza glabra, Poria, Platycodon, Chrysanthemum, Dong Gua Pi, etc. 5.Qi and Yin deficiency (recovery period) Evidence: tinnitus and dizziness, shortness of breath and weakness, spontaneous sweating and night sweating, heat in the hands and feet, dry mouth and tongue, red tongue with little moss, thin pulse. Treatment : Nourishing Yin, tonifying the spleen, clearing heat and stopping bleeding, adding Radix Codonopsis, Astragalus, Poria, Dioscorea, Kuncao, Plantago ovata, and Bupleurum. Western medical treatment 1, desensitization therapy; 2, application of glucocorticoids such as prednisone, methylprednisolone, etc.; 3, immunosuppressants cyclophosphamide, azathioprine, mortification, tacrolimus, etc.; 4, blood activation and anticoagulation with double mydriasis, heparin, simethicone, etc. Western medicine treatment is mostly based on the type of kidney biopsy pathology to select the above drugs.
