Zhi Shui Liver Clear Charge for drug-induced liver damage

  The author treated 30 cases of drug-related liver function abnormalities due to anti-TB and antiepilepsy with Zishui Qinghepatic Flush with satisfactory results, which are reported below.  1. Clinical data Among the 30 cases, there were 18 males and 12 females; age ranged from 23 to 54 years; 27 cases of tuberculosis and 3 cases of epilepsy. The liver function was normal before treatment of the primary disease (including 6 cases of chronic hepatitis B patients). The observed cases received anti-TB (each group contained 3 or more anti-TB drugs that were damaging to the liver) or antiepileptic treatment (all 3 cases contained 2 antiepileptic drugs that were damaging to the liver), respectively. The shortest period of abnormal liver function appeared in 1.5 months and the longest period in 4 months of anti-TB treatment, and all three anti-epileptic patients developed symptoms of liver damage after 6 months.  Liver function abnormalities were mainly manifested by varying degrees of elevated serum alanine transaminase (ALT) (maximum 3334.0 nmol・s/L, minimum 1533.6 nmol・s/L), with or without varying degrees of elevated glutamate transaminase (AST) and total bilirubin (Tbil). There were 16 cases of dysesthesia, 18 cases of fatigue, 13 cases of poor appetite, 4 cases of dizziness, 10 cases of abdominal distension, 8 cases of nausea, 2 cases of vomiting, 21 cases of dry and bitter mouth, 16 cases of heat in the heart of hands and feet, 10 cases of red tongue, 8 cases of greasy moss, 4 cases of fine or slippery pulse, and 4 cases of no obvious symptoms.  2.Treatment Method: Treatment with Zhi Shui Qing Liver Punch. The basic formula consists of 40g each of Radix Rehmanniae, Radix Rehmanniae, Cornus Officinalis, Cornu Cervi Pantotrichum, 30g each of Zeligia, Peony Peel, Poria, 50g each of Chai Hu, Zizyphus, Gardenia, Radix Paeoniae, Radix Angelicae Sinensis, etc. The above medicines are mixed and powdered into 1 ingredient, which is taken with warm water after meals, 10g (about 1 tablespoon) each time, 3 times a day. Addition and subtraction: add Yanhuosuo, Qingpi, Xiangxian, Yujin for dysmenorrhea; Buddha’s hand, Jiao Sanxian for poor appetite; Chrysanthemum, Shijiazhuang, raw oyster for dizziness; Mu Xiang, Citrus aurantium, Chen Pi for abdominal distension; Tortoise board, Bejia, Zihehe Che for hand and foot heart fever; Huang Lian, Wu Zhuyu, Zhu Ru, Chuanzi, Gentian Cao for nausea, vomiting, dry and bitter mouth; Wu Wei Zi, Yin Chen, Hawthorn, Adzuki bean for higher ALT. The dosage of drugs added or subtracted depends on the severity of the disease and symptoms. In all patients, only single ALT mildly elevated (1533.6~1800.4nmol・s/L), must be 2 weeks after re-checking ALT, did not decline before giving Chinese medicine treatment, a total of 30 cases, of which 6 cases of hepatitis B patients (all suffering from tuberculosis) found liver function abnormalities immediately after giving Chinese medicine bulk increased dosage, 15g each time, punch. This formula is composed of Liu Wei Di Huang Tang and Dan Yuan Yuan Yi Yao San, which nourish the liver and kidney, nourish yin and clear heat, dredge the liver and calm the mind. Consumptive disease and epilepsy are chronic diseases that are long-lasting and difficult to be cured, with long duration of anti-TB or anti-epileptic drug treatment and slow effect.  According to Chinese medicine, this formula is used to treat the symptoms of drug-related liver damage, and the formula is consistent with the evidence, and is effective when added and reduced according to the symptoms. I have changed the formula from a soup to a powder, which is convenient and inexpensive to take, and its efficacy will not be reduced by the change in dosage form, not to mention the efficacy of anti-TB or antiepileptic drugs. I also use this formula to treat patients with chronic type B active hepatitis, only increase the dose (15~20g each time), the enzyme-lowering and liver-protecting effect is obvious, and also has a preventive effect on early cirrhosis.
