Since most liver cancers are already in the middle and late stages at the time of diagnosis and lose the opportunity of surgery and interventional treatment, coupled with the fact that liver cancer is insensitive to systemic chemotherapy and has low efficacy, a significant number of patients choose or are considering whether to use Chinese medicine for treatment. Therefore, it is very important to understand the role of TCM in the treatment of liver cancer, especially advanced liver cancer. The focus of TCM treatment for liver cancer is not on tumor shrinkage, but on improving patients’ symptoms, reducing pain and prolonging survival time. TCM treatment has certain advantages in improving the survival quality and prolonging the time of patients with advanced liver cancer. To summarize, the prescription principle of TCM for liver cancer treatment consists of three parts: “identification” medicine (about 40%) + “disease identification” (anti-cancer) medicine (about 40%) + “symptomatic” medicine (about 20%). (about 20%). The so-called “evidence-based” medicine is the essence of Chinese medicine and the basis of prescription treatment. According to the different clinical manifestations collected from the four diagnoses, TCM patients are divided into the following four types of symptoms, and different treatments and prescriptions are adopted respectively. 1. Liver-depression and spleen-deficiency type: Although the tumor is advanced (spread and metastasis) according to western medical examination results, patients of this type generally perform well, with almost no special symptoms or only mild weakness, decreased appetite and diarrhea. The treatment should be to de-stress the liver and strengthen the spleen and regulate the qi. The identification prescription is Chai Hu Dredging Liver San plus or minus. 2. Qi stagnation and blood stasis type: Patients of this type are characterized by pain in the liver area and huge lumps. The treatment should be to activate blood circulation and eliminate blood stasis, move Qi and relieve pain. The prescription is Sui Yu Tang under diaphragm with addition and subtraction. 3. Damp-heat accumulation type: Patients of this type are characterized by skin and sclera jaundice. The treatment should be to clear heat and dampness, dredge the liver and bile. The prescription is Yin Chen Artemisia Tang combined with Gentian Diarrhea Liver Soup plus or minus. 4. Liver and kidney yin deficiency type: this type of patient is characterized by a long duration of illness, a thin body, a red tongue with little coating and a thin pulse. The treatment should be to nourish the liver and kidney. The prescription is Consistent Decoction with added flavor. The “disease” mentioned here is the “cancer” itself. If the prescription only identifies the evidence but not the disease, the anti-cancer effectiveness of the prescription will not be enough. In other words, a good prescription for liver cancer treatment should be based on evidence-based treatment, and some herbs with certain anti-cancer effects should be selected for “disease identification” (anti-cancer) treatment. Because these herbal medicines have similar anti-cancer rent as western chemotherapy drugs, they can act directly on tumors to inhibit the growth and proliferation of tumor cells or kill tumor cells. At present, the treatment methods of TCM disease identification therapy include the following categories. Acute seeds, stellaria, shougong, whole scorpion, centipede, strychnine, dew beehive, etc. 2, softening phlegm and dispersing knots: commonly used in the class of half asia, tiannanxing, raw oyster, turtle nail, cat’s claw, corrugated seed, seaweed, kombu, yellow medicine seed, etc. 3, tonic class: ginseng, astragalus, atractylodes, angelica, aconite, reishi, rehmannia, longan, etc. With the above two kinds of therapeutic drugs in the prescription of Chinese medicine, the “actual” problems of liver cancer patients, such as loss of appetite, abdominal distension, diarrhea, etc., often cannot be solved in the short term, as well as the most “unpleasant” problems. The “hardest” problems, such as ascites, jaundice, pain, etc. 2.Control ascites DD to improve the quality of life Besides the above identification and treatment, a variety of methods can be adopted for the comprehensive treatment of ascites in liver cancer. Intra-abdominal drug infusion: after discharging ascites, dilute 20ml of traditional Chinese medicine arsenious acid with 40ml of saline and inject into the abdominal cavity, and let the patient change the position once every 15 minutes after drug injection, which is more effective. 3.Control cancer pain DD to improve life quality: Patients mostly belong to qi stagnation and blood stasis type in hepatocellular carcinoma identification classification, and the medication is prescribed in qi stagnation and blood stasis type in identification treatment. In addition to the “three steps” and “timely, individualized and oral and external administration” of Western medicine, the unique features of Chinese medicine treatment include: various kinds of topical Chinese medicine creams, including Putuo Cream, Liver Cancer Pain Relief Cream, Anti-accumulation Pain Relief Cream, Ru Yi Jin Huang San, Toadstool Babu Cream, Pain Relief Cream, etc. (the latter four are in-hospital preparations of Guangdong Provincial Hospital of Traditional Chinese Medicine). Acupuncture and moxibustion to relieve pain: acupuncture points that can be used to regulate qi and activate blood circulation to relieve pain include Shimen, Zhangmen, Zhigou, Yanglingquan, Foot Sanli, Taichong, and Aye points. 4.Control jaundice DD is the vane of liver cancer prognosis. Patients with advanced liver cancer are most prone to jaundice, and once it occurs, it mostly indicates poor prognosis. The reason is that jaundice often reflects the poor liver function of the patient, which in addition to its poor prognosis, also limits the implementation of other treatments. Therefore, the ability to control the jaundice and make it subside often affects the survival of the patient. In this regard, in addition to the above-mentioned prescriptions, TCM practitioners also need to target “jaundice” by draining the liver, strengthening the spleen, and resolving dampness and diuresis, using formulas such as Yin Chen Jie Fu Tang and Yin Chen Artemisia Tang with Tian Ji Huang, Yu Jin, and Tiger Balm.
