Functions:
1. Steroidal gallbladder stones – must be stones that can be penetrated by X-rays. Also, the gallbladder must have normal contraction function;
2. Bile depression liver disease (e.g. primary biliary cirrhosis);
3. Biliary reflux gastritis.
Usage and dosage:
1. Steroidal gallbladder stones and biliary congestion liver disease
Take with a little water at regular intervals. 10mg/kg at a time according to body weight.
Lithotripsy: usually 6-24 months. If stones do not become smaller after 12 months of administration. Discontinue. The results of treatment will be determined by ultrasound or X-ray every 6 months.
2. Bile reflux gastritis
Swallow with water at night before going to bed. Must be taken regularly. One capsule (250 mg) once. Once a day. Usually taken for 10-14 days. Follow your doctor’s instructions for continued use.
Adverse effects:
Gastrointestinal disturbances: In clinical trials. Reports of loose stools or diarrhea are common with treatment with ursodeoxycholic acid. In the treatment of primary biliary cirrhosis. Severe right upper abdominal pain is rare.
Hepatobiliary dysfunction: When treated with ursodeoxycholic acid. Calcification of gallstones is rare. When treating advanced primary biliary cirrhosis. Loss of cirrhosis is rare. Partial recovery after cessation of treatment.
Allergic reactions: Urticaria is very rare.
Precautions:
Ursodeoxycholic acid capsules must be used under medical supervision.
The treating physician must check the patient’s liver function parameters such as AST (SGOT), ALT (SGPT) and gamma-GT every 4 weeks for the first 3 months of treatment. The liver function parameters should be checked every 3 months thereafter.
In order to evaluate the effect of treatment. Early detection of calcified gallstones. Depending on the stone size. 6-10 months after the start of treatment. Gallbladder Χ-ray (oral cholecystography). Radiographs are taken in the standing and lying position (ultrasound monitoring).
Contraindications:
Ursodeoxycholic acid capsules are contraindicated in the following cases:
Acute cholecystitis and cholangitis.
Obstruction of the biliary tract (common bile duct and cystic duct).
If the gallbladder cannot be seen under X-ray. Calcification of gallstones. Ursodeoxycholic acid capsules should not be used if the gallbladder does not contract properly and if there is frequent biliary colic.
Use in pregnant women:
Ursodeoxycholic acid has been found to be embryotoxic in early pregnancy in animal studies. There is a lack of experimental data in the first trimester of human pregnancy. Ursodeoxycholic acid capsules should be used in women of childbearing age only after safe contraceptive measures have been taken. Before starting treatment, it is important to exclude that the patient is pregnant. For safety reasons, ursodeoxycholic acid capsules should not be taken during the first trimester of pregnancy. Although there are no data to suggest that ursodeoxycholic acid passes into breast milk, it is recommended that ursodeoxycholic acid capsules not be taken during breastfeeding.
Medication for Children:
Take according to weight and medical condition, or as directed by a physician.
Geriatric Use:
Use with caution in geriatric patients.
Drug Action:
Ursodeoxycholic acid capsules should not be taken concomitantly with caurethamine (cholestyramine), colestipol (cholinesterase), aluminum hydroxide and/or aluminum hydroxide-magnesium trisilicate, as these drugs can bind to ursodeoxycholic acid in the intestine, thereby impeding absorption and impairing efficacy. If it is necessary to take any of these drugs, ursodeoxycholic acid capsules should be taken two hours before or two hours after taking the drug. Ursodeoxycholic acid capsules can increase the absorption of cyclosporine in the intestine. Patients taking cyclosporine should have their cyclosporine serum concentration monitored and the dose of cyclosporine taken should be adjusted if necessary. Ursodeoxycholic acid capsules can reduce the absorption of ciprofloxacin in individual cases. Ursodeoxycholic acid may induce the drug metabolizing enzyme cytochrome P450 3A4, based on reports that ursodeoxycholic acid decreases the peak plasma concentration (Cmax) and area under the curve (AUC) of the calcium antagonist nifedipine, as well as one case of interaction with aminophenazone (reduced therapeutic effect) and the results of in vitro and in vivo studies. The dose of ursodeoxycholic acid should be adjusted if necessary.
Pharmacological effects:
It promotes the secretion of endogenous bile acids, reduces reabsorption, antagonizes the cytotoxic effects of hydrophobic bile acids, protects hepatocyte membranes from cholesterol stones, and has immunomodulatory effects.
