What should we do if we have an acute attack of gout?

  Gout is a common disease caused by disorder of purine metabolism, excessive production and/or reduced excretion of uric acid, resulting in increased uric acid in the blood and deposition of uric acid salts in joints or other parts of the body. We found that the prevalence of hyperuricemia can be more than 10% in Guangdong, and it is a really common and frequent disease.  The course of gout can be roughly divided into the acute phase (also known as the attack phase) and the remission phase (also known as the intermittent phase), the remission phase is almost asymptomatic; however, in the acute phase, the sudden onset of single joint redness, swelling and heat pain, but the pain is so shocking that it can cry to the gods and the earth, in the words of a patient: “Gout, gout, the pain is really like crazy.” It is clear that the pain of gout is not trivial.  In the acute attack period of gout, mainly rely on drug therapy; treatment drugs are probably the following: 1, colchicine, is a familiar drug for many people, generally start eating 2 tablets (0.5mg/tablet), after every 2 hours plus 1 tablet, and so on, eat until the pain relief, up to a day can eat 10 tablets. Why can’t you take several tablets at a time, but at intervals of several hours? The main reason is that colchicine has relatively large side effects, the biggest one being diarrhea. Many patients just take 2 or 3 tablets, and before the pain is relieved, the diarrhea starts. Therefore, with colchicine, it is usually necessary to stop the drug when diarrhea occurs. As for other side effects, it is not possible to take colchicine during pregnancy preparation, and long-term use of colchicine requires attention to blood tests.  2, non-steroidal anti-inflammatory drugs (cyclooxygenase inhibitors), that is, we usually say painkillers, many kinds, including nimesulide, diclofenac, ibuprofen, meloxicam, ciloxib and etoricoxib and so on, and this, there is a class of cyclooxygenase-2 specific inhibition of painkillers, such as etoricoxib, pain relief effect is very good, stomach damage is relatively small, currently in the clinical application of the most. However, we should still pay attention to the fact that patients who have previously had gastric ulcer, gastric bleeding, hypertension, coronary heart disease or heart failure must use the medication under the guidance of a doctor and not blindly use painkillers on their own.  3, hormonal drugs, I guess we heard hormones are afraid, but this is a very good class of rapid pain relief drugs, when the previously mentioned colchicine and painkillers do not work well, generally will consider using hormones. And it is best if it can be injected locally in the joint, with fast pain relief and little side effects. I remember a patient who had a gout attack in the knee joint. When he visited the clinic, his joint was so red and swollen that he took colchicine and etoricoxib, and also hormones, which were not effective. Oral or intravenous use of hormones has many side effects, especially gastrointestinal reactions, so all patients with previous gastrointestinal ulcers and gastric bleeding should use hormones with caution; moreover, hormones have an effect on blood sugar and blood pressure, so patients with hypertension and diabetes also need to weigh the pros and cons of using the drugs and use them with caution.  External pain relief creams or Chinese ointments are also a way to relieve pain. However, since most gout patients have severe localized redness, swelling and pain in the joints, the effect of topical creams is limited, and it is necessary to pay attention to whether the creams are allergic, and if allergic, it is necessary to stop using them in time.  These drugs are appealed, in the process of use, it is recommended that patients should go to the rheumatology department of a regular hospital to see, it is best not to use the drugs at their own discretion.  In the drug treatment, we can also reduce joint activities, ice to assist in the treatment, because gout in the attack, local redness, swelling and heat pain, ice can reduce the temperature to relieve redness and pain; remember not to apply hot compresses at this time, heat will only aggravate the pain. At the same time, strenuous exercise and local joint massage are not allowed during the gout attack period, as they will aggravate the disease, and only after the joint pain is mostly relieved can you exercise and move your joints properly.  Finally, when the pain is mostly relieved (no pain when resting or walking slowly, no pressure pain in the joints, but still mild pain from strenuous exercise), you can gradually reduce the dosage until you stop using pain medication, and then start regular uric acid reduction therapy.
