No need for biliary drains after choledochotomy for lithotripsy

  Treatment methods for simple common bile duct stones: 1, gastroenterologist via EST to retrieve stones, in this regard, the Department of Gastroenterology of our hospital has done a lot of work and enjoys a good reputation in northern Jiangsu Province; 2, laparoscopic choledochotomy to retrieve stones and T-tube drainage, which are routinely carried out in all tertiary hospitals; 3, open abdominal surgery to perform choledochotomy to retrieve stones and T-tube drainage, which is the most traditional and classic procedure. 2 gallbladder stones combined with Common bile duct stones: laparoscopic surgery and open abdominal surgery are generally used. All the above different methods have their indications and eventually they all have to be performed with bile duct drains and placed for 2-4 weeks. Hospitalization time, cost, and physical and mental pain all rise accordingly. Due to the rapid development of modern surgical techniques and surgical instruments, the traditional surgical treatment concept and treatment methods have changed dramatically. In this case, after cholecystectomy and choledochotomy, intraoperative cholangiogram was performed to confirm that there were no residual stones, and 4″0″ slip sutures were used to close the common bile duct wall continuously without biliary drains. As a result, the hospital stay, cost, and patient pain are minimized.  This method requires a high level of case selection, surgical suturing technique, and sutures. We have implemented 15 patients, all of whom are expected to recover and be discharged from the hospital and live and work in good health. All surgical operations should be performed to ensure patient safety, to achieve the exactness of efficacy, the pursuit of minimal surgical trauma and the exquisite realm of surgical operation. The pursuit of formalistic perfection must not be overdone.
