Chronic pain is a relatively common problem, and it is important to remind people that about 40% of chronic pain patients will develop depression, with an incidence four times higher than that of the general population. However, many patients only emphasize somatic pain sensations and attribute some depressive symptoms to the clinical manifestations of somatic diseases, repeatedly tossing and turning to general hospitals, delaying the condition and missing the opportunity for early treatment. This was the case of a female patient who came to the clinic two days ago. 1.5 years ago, she developed headache, neck and shoulder pain, vague stomach pain and other discomfort. After repeated visits to local and Beijing hospitals, many tests were done and no abnormalities were found except for superficial gastritis. He was given targeted treatment and local joint closure for several months, but with little success, the pain and discomfort were still obvious and he could not live and work normally. It was not until six months ago when the patient had insomnia, depression, feeling uninterested in doing anything and worse than death that he first consulted a psychiatric hospital and was diagnosed with depression and given antidepressants combined with “Valium” medication. The patient was able to perform household chores. Most chronic pain is one of the early symptoms of depression and has the following characteristics: the location and degree of pain are often not consistent with the underlying disease, or there is no organic lesion to explain the pain symptoms; the onset, development, persistence or aggravation of pain is closely related to psychological factors such as anxiety, depression and other emotions, which are mutually reinforcing; the location of pain is often indeterminate and can be multiple locations; the nature of pain is diverse and can be persistent dull pain, dull pain, painful pain, painful pain and painful pain. The nature of pain is varied and can be persistent dull pain, severe eruptive pain and intermittent electric shock-like pain. Therefore, patients with pain in the above conditions should also consider psychiatric consultation when focusing on somatic disorders.
