Objective To explore the correct management of perioperative period of dual-donor living donor liver transplantation and its effect on the recovery of donor and recipient. Methods To analyze the surgical indications, graft selection, surgical methods, and postoperative liver function recovery and CT examination results of one case of dual-donor living liver transplantation between adult-children, as well as the postoperative liver function recovery and CT examination results of the donor and the recipient. RESULTS The liver function of the donor was completely normalized on the third day after the operation, and there were no obvious surgical complications. Recipient’s recovery was also quite smooth, without obvious surgical complications; there was mild rejection 2 weeks after surgery, which was controlled after adjusting the immunosuppressive regimen. Conclusion Although dual-donor living donor liver transplantation is usually used in adults, it has to be considered in the increasing number of overweight minor recipients. Proper perioperative management can effectively prevent small volume syndrome and maximize donor safety, minimizing surgical risk for the entire family.
