Am I really just depressed?

  In our daily habits, people are more likely to remember negative emotions or negative events, and less likely to remember positive events or enjoyable things! So one tends to experience and easily remember when one feels very bad emotionally; for happy moments but easily forget, or feel that this is the way life should be. In our clinical process, we found that when a person has depression, it is relatively easy for the person and his family to find out and pay attention to it; but for manic episodes, especially light manic episodes, it is easy to ignore them, and even if the person and his family feel something different or feel something wrong, they will not think that it is a kind of pathology, and some people will even think that it is better to be in that state.  In the psychiatric clinic, in the past, the ratio of depression and bipolar disorder was about 10:1, but now the study found that the ratio of depression and bipolar disorder is about 1:1. Why is the difference between the ratio before and after so big? The reasons for this are manifold. First, when patients and their family members tell doctors about their condition, they often only talk about unhappy mood, not wanting to talk, not wanting to move, fatigue and weakness, pessimism, low self-esteem, guilt and self-blame; while for the opposite, inconsistent with their personality, excitement, talking a lot, increased self-confidence, energy, being busy, easily losing temper, needing less sleep, liking to spend money, doing things Manic (light manic) states such as lack of consideration or becoming bolder are not realized to be pathological and are not told to the doctor. When the doctor asks the patient and family members about it, the patient or family members think that it is quite normal or do not remember it, so they are not able to respond to the doctor completely about the condition. Of course, another important reason is that medical awareness of this disease has increased, the level of diagnosis has improved, and doctors are paying more attention to the identification of bipolar disorder than ever before.  What are the negative consequences if bipolar disorder is not recognized in time? One situation is that, during the treatment of depression, or after getting well for a period of time, family members find that the patient becomes too excited, busy all day long, very good spirit, big temper, and spend a lot of money! This is turning into mania; another situation is taking many kinds of antidepressants, but the effect is always bad; now a study also found that bipolar disorder can be timely diagnosed and treated, but also can reduce the chance of suicide.  The identification of bipolar disorder is actually difficult for a number of reasons: 1. Incomplete reporting of the medical history, noting only the reaction to depression, but neglecting the reporting of excitement and self-confidence that is inconsistent with the usual personality; 2. Too busy in clinical work, doctors do not have enough time to find out the light manic episodes that the family and the patient did not notice by asking a variety of different questions when taking the medical history The occurrence of bipolar disorder is a longitudinal process, only depressive episodes in the past and present, while manic episodes appear sometime in the future, so it cannot be detected now; 4. The disease is always more complicated, and the diagnostic criteria are strict, for some atypical cases, it needs doctors, patients and family members together to observe and judge, and even need to continue to observe and make judgment according to the development of the disease.  Therefore, depressed patients and their families should pay attention to the comprehensive report of medical history during the consultation process, taking the patient’s usual personality as the baseline, considering the report of the situation when the mood is low, and also paying attention to the report of whether there is any excitement that is inconsistent with the personality, so that the doctor can make a judgment. In the process of treatment, or in the process of continuing to take medication after the condition is under control, it is important to pay attention to the presence of excitement inconsistent with personality and, if it occurs, to promptly contact one’s supervising physician for a timely review and challenge of the treatment plan.
