As the fertility rate of China’s population continues to decline and life expectancy per capita continues to increase, the process of population ageing is continuing to intensify throughout the country. The latest statistics show that, by 2015, China will have more than 200 million older persons over 60 years of age. Creating a healthy aging environment can reduce the demand for health resources by the elderly, while at the same time improving their quality of life and enhancing their sense of well-being. I. Impact of prostate diseases on the health of elderly men The prostate gland is a male-specific organ and the largest accessory sex gland in the male reproductive organs, with important roles in exocrine and endocrine secretion, control of urination and semen transportation. Prostate diseases (mainly including prostate hyperplasia and prostate cancer) are one of the major factors affecting the health and quality of life of elderly men. In China, the incidence of prostatic hyperplasia is more than 80% in older men over 60 years old, while the actual clinical diagnosis of prostatic hyperplasia is much lower than its incidence, which indicates that the harm of prostatic hyperplasia to older men has not been paid enough attention. The common clinical manifestation of prostate hyperplasia is urinary symptoms, which is caused by the extrusion of the hyperplastic prostate tissue on the urethra. We can often hear patients complain that their urination increased, first for the increase in the number of nocturia, waiting for urine later as the hyperplasia of the prostate gland on the urethra extrusion of the intensification of the patients feel urination effort, sometimes need to hold their breath to increase abdominal pressure to help urination, and the onset of urination is delayed, urination time is long, the urine line becomes thin and weak. Prostatic hyperplasia greatly affects people’s quality of life. If not treated in time, it may also lead to acute and chronic urinary retention, complicate urinary tract infections, bladder stones, hematuria, affect sexual function, and lead to hydronephrosis or even renal failure, uremia, and so on. In addition, the incidence of prostate cancer in Europe and the United States has been ranked first in the incidence of male malignant tumors. With the aging population in China and the change of people’s living habits and diet structure, the incidence rate of prostate cancer has been rising year by year. By 2008, in the more developed cities of Beijing, Shanghai and Guangzhou, the incidence rate and mortality rate of prostate cancer had jumped to the first place of male genitourinary tumors. According to clinical statistics, about 60% of prostate cancer patients are already in advanced stage when they are diagnosed, and 22% of them have distant metastasis, with poor treatment effect and extremely poor long-term prognosis. Therefore, early detection of prostate cancer is very important to improve the treatment effect of the disease and reduce the mortality rate of prostate cancer patients. What bad habits hurt prostate? According to the survey, the prevalence of prostate disease in smokers is 1 to 2 times higher than that in non-smokers. The more you smoke, the greater the prostate victimization. The second is afraid of dietary disorders: often eat spicy, acidic food and drink strong alcohol, can cause blood vessel expansion, prompting the prostate gland congestion. Fear of cold: the prostate gland is rich in adrenergic receptors, cold is very easy to cause sympathetic excitation, leading to gland contraction, so that the urethra pressure increases, affecting urination; and urinary difficulties, but also on the prostate gland will have a negative impact on the vicious circle of the prostate gland can be lesions. On the contrary, the sexual desire of the person for some reason can not be normal excretion, to the prostate secretion of a large number of “hoarding”, a long time lead to excessive expansion of the prostate gland and congestion, can also trigger inflammation. In addition, extracorporeal spermatozoa, sexual intercourse interruption can also make the prostate gland congestion and swelling and cause inflammation. Fear of constipation: constipated people rectal accumulation of large amounts of feces, will aggravate the neighboring prostate gland congestion. At the same time, constipated people tend to defecate with too much force, abdominal pressure increases, pressure on the prostate, can make the urethra thin, obstruction of urination, the prostate health is not favorable. Six afraid of infection: a patient went to the hospital a few times after the catheter, they bought two catheters and a bottle of glycerin, a little urination is not fast, they inserted the catheter catheterization, the results of the more inserted the more diligently, and finally even pulled out. After laboratory tests, there are a large number of purulent white blood cells in the urine. Doctors, their own catheters lead to infection, aggravated prostate enlargement, leading to more difficult urination. Seven afraid of extrusion: sedentary or often ride a bicycle so that the prostate gland for a long time by extrusion, resulting in poor local blood circulation. Especially when riding a bicycle, the car seat and perineum produce friction, stimulation of the upper part of the urethra and prostate and other places, promote subcutaneous tissue chronic hyperplasia, hardness, enlargement, or even inflammation, oppression of the urethra and prostate, easy to cause prostate disease and urinary difficulties. Eight afraid of holding urine: often artificially hold urine, can make the bladder full and distended, resulting in urinary weakness, aggravate the symptoms of prostate hyperplasia. Third, the prostate disease diagnosis and treatment of the misunderstanding: 1, taboo medicine Survey shows that more than half of the men are or have been suffering from male genitourinary diseases, seriously affecting physical and mental health and quality of life. Genitourinary diseases are often more private, patients and there are many misconceptions, some of which will be genitourinary diseases and sexually transmitted diseases, resulting in a lot of people are ashamed of the disease, unwilling to go to the hospital for treatment. Unbeknownst to them, this may not only delay the treatment, make the condition worse, but also very easy to induce psychological problems, and even life-threatening. 2, prostate hyperplasia symptoms is a very natural physiological aging phenomenon, do not worry too much The survey found that the prevalence of men over the age of 50 is about 50%, the prevalence of men over the age of 60 is about 60%, and the prevalence of men over the age of 80 is almost 100%. In the report, the Chinese Medical Doctor’s Association suggests that men over the age of 50 should go to the urology department of the hospital for regular screening of prostate enlargement risk, even if they do not have symptoms of BPH yet. Once any risk of prostate enlargement is detected, it is important to treat it early. For men who have already found symptoms, it is even more important not to delay again and again, so as not to miss the condition. 3, prostate enlargement is a minor problem, should prioritize the treatment of other diseases Prostate enlargement is a benign lesion. It is a benign disease that usually has a slow course, and if it doesn’t cause obstruction, it is almost asymptomatic and has little effect on life expectancy. However, when prostate enlargement reaches a certain level and causes urinary symptoms, especially difficulty in urination and urinary retention, it will have a negative impact on health, often leading to hernia, hemorrhoids, hematuria, bladder stones, recurrent urinary tract infections, hydronephrosis, uremia and other complications. If urinary symptoms are not treated and relieved in a timely manner, frequent nocturia will seriously affect sleep and rest, and thus aggravate hypertension, coronary heart disease, diabetes, heart failure, cerebrovascular disease and other common diseases in the elderly, forming a vicious cycle that directly affects the life expectancy of the elderly. 4, prostate hyperplasia patients stop taking medication without authorization In those patients who have received treatment, when the symptoms have been initially improved, they stop taking medication without authorization, resulting in recurrence or deterioration of the condition. The medication for prostate enlargement is the same as the medication for high blood pressure and diabetes, and it needs to be taken for life. 5, prostate enlargement “everything” prostate enlargement is not everything, in fact, most of the enlargement does not need surgery. 6. Confusing prostate enlargement and prostate cancer Early symptoms of prostate cancer are not obvious, and sometimes show symptoms of urinary obstruction similar to prostate enlargement, such as progressive dysuria, thinning of the urine stream, bifurcation of the urine stream, prolongation of the urinary process, frequent urination, urinary urgency, increased nocturia, and a feeling of incomplete urination. Some patients may have pain, loss of weight, fatigue, loss of appetite, but these are usually signals of advanced tumors.Since the 1990s, serum prostate-specific antigen (PSA) testing has been widely carried out in Europe and the United States, which has greatly increased the detection rate of early prostate cancer patients. As the most sensitive and widely used tumor marker for prostate cancer, many hospitals in China have also carried out the PSA test, so that gay men can conveniently rule out the possibility of cancer by donating only a few milliliters of blood. The whole screening process usually takes about 5 minutes and is painless. Experts suggest that men over 50 years old should have PSA test at least once a year, and for those with family history, the test age should be advanced to 45 years old. 7.Misunderstanding of Prostate Cancer “I thought cancer is just a ‘distant legend’, but there are more and more cancer patients around me”, many people have such a feeling. Some patients or their family members are overly afraid of malignant tumors, and they think that malignant tumors are incurable, which is tantamount to being sentenced to death. In fact, it is not. The World Health Organization pointed out in its report that 1/3 of cancers can be prevented, 1/3 of cancers can be cured through early detection, diagnosis and treatment, and 1/3 of cancers can be treated appropriately to prolong life and improve life quality. The opinion of authoritative experts is that cancer is not so terrible as long as it is correctly recognized and scientifically prevented. The cure rate of prostate cancer is quite high as long as it can be detected and diagnosed early. According to the statistics of our hospital in the past 20 years, the 5-year survival rate of patients with limited prostate cancer who have undergone radical surgery is close to 100%. Therefore, early prostate cancer is a completely curable tumor.
