Recently, a case of extremely difficult hepatic hemangioma was successfully resected in Hepatobiliary Ward 7-8 of our hospital. The patient was 49 years old, and the CT and MRI examination showed that the hepatic hemangioma was about 12 cm in diameter, located between the second and third portals of the liver, encircled by the left hepatic vein, right hepatic vein, and inferior vena cava, and the middle hepatic vein passed through the hemangioma, and the tumor took up the whole section VIII, section IVa, and all the caudal lobe, and involved part of section VII, and it was thought that there was no possibility of resection and the risk of resection was too high, and interventional therapy was recommended. “After consulting with several hospitals, it was concluded that there was no possibility of resection and that the risk of resection was too great, and interventional therapy was recommended. The patient underwent two interventional therapies in May and June 2013, and the CT review in August and December 2013 and April 2014 showed that the hemangioma had shrunk, but the result was unsatisfactory. From October 2014, the patient developed low-grade fever, chills, cold sweats, back swelling, shortness of breath, which gradually worsened, and she could not do housework. The patient suffered in pain and was disappointed in further treatment. The patient’s family was unwilling to give up, and after many inquiries on the Internet, in March 2015, he came to my expert clinic. After careful examination of the patient and related information, it was found that although the location of hemangioma was more special, the patient was younger, with good physical condition and basically normal liver function, more importantly, although the hemangioma was a benign tumor, the patient was tortured to lose confidence in life, and his spirit was in the verge of collapse, and he was determined to have a surgery. After careful consideration, the patient was admitted to the hospital. Peng Chuang of Hepatobiliary Surgery Department of Hunan Provincial People’s Hospital Under the attention, care and kind guidance of Professor Wu Jinjiu, Chief Scientist, and Professor Jiang Bo, Director of Hepatobiliary Hospital, the Hepatobiliary Surgery team conducted serious preoperative discussions, formulated a thorough treatment plan, and formed a surgical team consisting of Peng Chuang, Li Ou, Sun Zengpeng, and Quan Yong, and carried out resection of segment Ⅷ of the liver, segment Ⅳa, and part of segment Ⅶ + total caudate lobectomy, with the whole tumor resected, and the middle hepatic vein completely preserved, as scheduled. The tumor was resected in one piece and the middle hepatic vein was preserved. After 5 hours of hard work, the surgical team successfully completed this difficult surgery, and under the careful treatment and care of the medical and nursing team of Hepatobiliary Surgery Ward 7 and 8, the patient was discharged from the hospital on the 8th day after the surgery without any complication. This surgery is a typical case among the many difficult surgeries performed by the Department of Hepatobiliary Surgery of Hunan Provincial People’s Hospital every year, and the success of the surgery means that the patient, who had been tortured by the disease to the point of losing confidence in life, started a new life again with confidence.
