Old man on “death row” begins new life

Due to its deep location, crowding of important organs, dense surrounding blood vessels, complex anatomical structure and narrow surgical space, the huge retroperitoneal tumor in the upper abdomen is regarded as an impenetrable forbidden zone by surgeons, and a large number of patients have to spend their last days in pain and suffering. The Hepatobiliary Surgery team of Hunan Provincial People’s Hospital, with a kind heart and a kind technique, has repeatedly entered the forbidden zone of life, aiming to seek hope for the patients with huge tumors in the retroperitoneum of the upper abdomen. In July this year, a 64-year-old man came to the expert clinic of Prof. Peng Chuang. The old man had intermittent abdominal pain 4 years ago, which could be relieved by himself and was not serious, so he did not pay attention to it. Starting from May this year, the patient had decreased appetite and frequent episodes of abdominal pain, and went to several hospitals for treatment. Imaging results showed that: a huge tumor was found in the posterior and inferior part of the pancreatic region, with a maximum diameter of more than 13cm; the tumor was not clearly demarcated from the duodenum and the head of the pancreas, and the tumor was extruding the inferior vena cava and the left renal vein, which flattened out like a line, and extruding the superior mesenteric artery, the venous veins, and the portal vein and causing them to be displaced; the tumor was partially wrapped around the superior mesenteric artery and vein. The tumor partially wraps around the superior mesenteric artery and vein. The abdominal artery trunk was mildly narrowed, and there was a thick mutated blood vessel at its root to supply blood to the tumor, and many blood vessels of the superior mesenteric artery and the left edge of the abdominal artery trunk were in the shape of a hugging ball to supply blood to the tumor, which made the tumor’s blood supply very rich and involved many important organs, which was called the “landmine in the forbidden zone of life”, and the doctors usually didn’t dare to touch it. What’s more, the patient is also a senior citizen, and any hemorrhage that occurs during the surgery may be directly life-threatening. However, surgery is the only effective way to improve the patient’s condition and quality of life. After a thorough evaluation of the disease and the patient’s condition, the patient was admitted to the hospital. Under the attention, care and kind guidance of Chief Scientist Prof. Wu Jinjiu and President of Hepatobiliary Hospital Prof. Jiang Bo, the Hepatobiliary Surgery team conducted serious preoperative discussions, formulated a thorough treatment plan, and formed a surgical team consisting of Peng Chuang, Yi Weimin, Li Guoguang, and Hu Weitao, and performed retroperitoneal resection of a huge tumor + anastomosis with proximal resection of the duodenum and jejunum for the patient as scheduled. After nearly ten hours of hard work, the surgical team successfully completed this difficult surgery. The patient’s postoperative pathology showed a huge retroperitoneal paraganglioma (13×8×7cm) with malignant potential. The patient was discharged from the hospital 12 days after the operation under the careful treatment and care of the medical and nursing team in the seventh ward of Hepatobiliary Surgery. This old man, who had been sentenced to death by many hospitals, started a new life with confidence.
