How can gallbladder stones be treated well?

Patient: I’ve had gallbladder stones for five years, and I heard a doctor say that there is no good way to remove gallstones, and that when the pain is too much, I should have my gallbladder removed surgically. Therefore, I have not cared about it, but just check the size of the stone every year, for four years it has been about 1.2cm, and sometimes I feel uncomfortable in my back, but it has never been a big pain. Recently, the stone suddenly became bigger (1.4cm), and I heard that gallbladder stones should be treated early, and it is better not to remove the gallbladder, even if it is cut, the stones will still grow in other parts. After checking the internet, I found that it is very confusing, saying that lithotripsy can remove stones and so on. But I think it’s time to pay attention to gallbladder stone treatment. Now, my liver is normal in size and shape, and the intrahepatic bile ducts are not dilated. The gallbladder wall is also normal and multiple stones are visible in the cyst. I am 36 years old, male and my body is otherwise normal. I would like to ask how to treat it. Shen Zhanlong, Department of General Surgery, Peking University People’s Hospital: There are many ways to treat gallbladder stones. But many of them are not very effective. 1, as you said a doctor said, “if the pain is too much, do surgery” is definitely not advisable. The reason is that emergency cholecystectomy surgery is risky and generally can only be done openly. Nowadays, we try to give conservative anti-inflammatory treatment to patients with acute cholecystitis, and then do laparoscopic surgery after the acute period, so the method you mentioned is definitely undesirable, or an old concept. 2. The opening of the bile duct to the intestine is very small (not as big as the urethral opening, so kidney stones can be broken), only 1-2mm, so the stones cannot fall down and accumulate in the bile duct, which will be very dangerous. 3, there is a possibility of growing stones in the bile duct again after gallbladder removal, but the incidence is very low, because the composition of stones in the gallbladder is different from the composition of stones in the bile duct. The incidence of long stones in the bile duct is low, but it cannot be completely excluded.4.Biliary stone retrieval, you can remove the stone to preserve the gallbladder, but the recurrence rate is relatively high, many of our patients come back for surgery after biliary stone retrieval surgery.5.At present, people mainly choose laparoscopic cholecystectomy, as long as this hospital has this technology.6.You yourself have uncomfortable symptoms now, and are multiple stones, there is an attack The risk of cholecystitis, if you have the desire to do surgery, I still recommend that you operate, laparoscopic cholecystectomy.
